Abstract(s)
A notificação de incidentes é uma ferramenta fundamental na identificação de riscos e vulnerabilidades, garantindo a segurança e a melhoria contínua nas instituições de saúde. Nos Cuidados de Saúde Primários, a subnotificação de incidentes é uma realidade, e ainda pouco explorada por estudos neste contexto.
Com estes pressupostos, desenvolveu-se um estudo que teve como objetivos: conhecer a perceção dos enfermeiros gestores/enfermeiros especialistas em funções de gestão sobre a notificação de incidentes nos Cuidados de Saúde Primários; identificar os fatores que influenciam essa notificação; descrever as estratégias utilizadas na gestão da notificação; e descrever como estes profissionais consideram ser a abordagem mais eficaz para melhorar a notificação de incidentes.
Trata-se de um estudo qualitativo, do tipo exploratório e descritivo, com recurso a entrevistas semiestruturadas para a recolha da informação, em que participaram doze enfermeiros que exerciam funções de gestão em contexto de Cuidados de Saúde Primários. Para o tratamento da informação recorremos à análise de conteúdo, segundo Bardin (2022).
A análise resultou em quatro categorias: perceção sobre notificação de incidentes, fatores que influenciam a notificação, estratégias de gestão da notificação e perspetivas de melhoria. Da primeira, foram identificadas quatro subcategorias: sistemas de notificação, de supervisão, de subnotificação e de encaminhamento para instâncias superiores. Os fatores que influenciam a notificação, incluiu cinco subcategorias: consciencialização, feedback, sobrecarga de trabalho, défice de conhecimento e medo de repercussões. Já as estratégias de gestão foram agrupadas em três subcategorias: participação no processo de trabalho, cultura de segurança organizacional e acessibilidade. Por fim, a perspetiva de melhoria destacou quatro subcategorias: formação dos profissionais, otimização das plataformas, política organizacional e ambiente de trabalho seguro e saudável.
É reconhecida a importância da notificação de incidentes para aumentar a segurança e a qualidade dos cuidados prestados. Assim, é essencial intensificar o investimento na promoção de uma cultura de segurança e de notificação de incidentes. O enfermeiro gestor desempenha um papel crucial na promoção de uma cultura de segurança, assegurando que as notificações sejam valorizadas e devidamente tratadas. O investimento em formação contínua e sistemas acessíveis é essencial para aumentar a eficácia da notificação de incidentes.
Incident reporting is a fundamental tool for identifying risks and vulnerabilities, ensuring safety and continuous improvement in healthcare institutions. In primary health care, the under-reporting of incidents is a reality, and one that has not yet been explored by studies in this context. With this in mind, a study was carried out with the following aims: to find out the perceptions of nurse managers and specialist nurses in management roles about incident reporting in primary health care; to identify the factors that influence this notification; to describe the strategies used to manage reporting; and to describe what these professionals consider to be the most effective approach to improving incident reporting. This is a qualitative, exploratory and descriptive study, using semi-structured interviews to gather information, from twelve nurses working in management roles in primary health care. Content analysis was used to process the information according to Bardin (2022). The analysis resulted in four categories: perception of incident reporting, factors influencing reporting, strategies for managing reporting and prospects for improvement. From the first, four sub-categories were identified: reporting systems, monitoring, under-reporting and referral to higher authorities. Factors influencing reporting included five subcategories: awareness, feedback, work overload, lack of knowledge and fear of repercussions. Management strategies were grouped into three subcategories: involvement in the work process, organisational safety culture and accessibility, and the improvement perspective highlighted four subcategories: training of professionals, optimisation of platforms, organisational policy and safe and healthy working environment. The importance of reporting incidents to improve the safety and quality of care is recognised. It is therefore essential to increase investment in promoting a culture of safety and incident reporting.
