Avaliação Económica da Utilização de um Sistema de Aquecimento Térmico no Contexto Perioperatório

Abstract(s)
A hipotermia inadvertida é uma complicação frequente em procedimentos anestésico-cirúrgicos (Muniz et al., 2014), que acarreta custos elevado para os serviços de saúde (Azenha et al., 2017), no entanto existem poucos estudos que comparem, do ponto de vista económico, diferentes dispositivos de aquecimento usados no contexto perioperatório. Os objetivos deste estudo são identificar e comparar os custos de duas alternativas de aquecimento térmico utilizadas no ambiente perioperatório.
Foi realizada uma avaliação económica do tipo minimização de custos, cuja metodologia adotada foi um estudo prospetivo e correlacional. Os dados obtidos resultam da consulta de documentação disponível de unidades hospitalares da zona norte do país, sob a perspetiva do SNS. A colheita da informação foi realizada segundo a abordagem bottom-up.
Foram identificadas as diferenças de custos entre os dois sistemas térmicos, o sistema por ar forçado e o sistema de isolamento térmico de três camadas, ao final do primeiro, quinto e décimo anos de utilização, levando em consideração o número de cirurgias em 2022, a depreciação anual de ambas as opções, a aplicação de taxa de atualização de 5% e a análise de sensibilidade.
O presente estudo foi desenvolvido reconhecendo que identificar os custos associados a ambas as opções constituiria um importante contributo para o conhecimento das vantagens e desvantagens da sua utilização, e para a decisão de uma escolha mais favorável do ponto de vista económico.
Este estudo revelou que, de entre as hipóteses de aquecimento usadas no bloco operatório, o isolamento de três camadas é a mais favorável para o SNS, ao fim do 1º, 5º e 10º anos de utilização
Non intended hypothermia is a common complication in anesthetical and surgical procedures (Muniz et al., 2014), this leads to high costs for healthcare services (Azenha et al., 2017), however, there are few studies that, over an economic perspective, put in comparison all the existent perioperative heating devices. The goals of this study are to identify and compare the costs of two thermal heating alternatives used in the perioperative environment. A cost-minimization economic evaluation was conducted. The adopted methodology was retrospective and correlational. Data was collected from a hospital in the northern region of the country, using primary data, from the perspective of the National Health Service. Information gathering followed a bottom-up approach. Cost differences between the two thermal heating systems were identified, the forced -air blanket and the three-layer thermal insulation system, on the first, fifth, and tenth years, taking into account the surgical volume in 2022, the annual depreciation of both options and the application of a 5% refresh rate and sensitivity analysis. This study was developed with the recognition that identifying the costs associated with both options would be a significant contribution to understanding the advantages and disadvantages of their use, and to the decision of a more economically favorable choice. This study revealed that among the thermal heating options used in the operating room, the three-layer insulation system is the most favorable for the National Health Service at the end of the 1st, 5th and 10th years of usage.
