Abstract(s)
Os Cuidados Paliativos destinam-se a pessoas com doenças incuráveis e progressivas e suas famílias, visando proporcionar cuidados de alta qualidade em toda a trajetória de doença, incluindo o fim de vida, focando-se na prevenção e alívio do sofrimento físico, psicossocial e espiritual, no sentido de melhorar a qualidade de vida da pessoa/família. A fase de Últimos Dias e Horas de Vida (UDHV) é pautada por uma deterioração progressiva e, por vezes, com sintomas associados, com necessidade de cuidados intensificados, incluindo medidas de conforto e suporte à família. O enfermeiro desempenha um papel crucial na prestação de cuidados devido à proximidade com a pessoa/família. Surge assim a questão de partida do Projeto de Aprendizagem: “Qual a intervenção do Enfermeiro Especialista no cuidado à pessoa em UDHV?”. O percurso realizado teve como finalidade desenvolver competências comuns de enfermeiro especialista e específicas em Enfermagem Médico-Cirúrgica na área de Enfermagem à Pessoa em Situação Paliativa e também de Mestre, e contribuir para melhoria da qualidade dos cuidados de enfermagem, tendo incidido na área do cuidado à pessoa nos Últimos Dias e Horas de Vida. O presente Relatório, que se insere no âmbito do 1º Curso de Mestrado em Enfermagem, na área de especialização em Enfermagem Médico-Cirúrgica, na área de intervenção à Pessoa em Situação Paliativa, apresenta uma descrição e análise fundamentada na literatura sobre as atividades e as competências desenvolvidas nos dois estágios, Ensino Clínico na Equipa Comunitária de Suporte em Cuidados Paliativos e na Equipa Intra-Hospitalar de Suporte em Cuidados Paliativos. Neste percurso foi utilizada a metodologia de Projeto. As estratégias utilizadas foram: consulta de documentos dos serviços; realização de reflexões críticas sobre a prática adotada; pesquisa bibliográfica; elaboração de uma Revisão Scoping; observação da prática clínica; prestação de cuidados; consulta de registos de enfermagem; elaboração de documentos de apoio à prática clínica; consulta de peritos; sessões de formação/ discussão às equipas. Com este percurso considero que atingi a finalidade a que me propus, encontrando-se explicito neste Relatório os resultados obtidos.
Palliative Care is intended for people with incurable and progressive illnesses and their families. It aims to provide high-quality care throughout the course of illness, including the end of life, focusing on the prevention and relief of physical, psychosocial and spiritual suffering, in order to improve the quality of life of the person/family. The Last Days and Hours of Life (LDHL) phase is marked by a progressive deterioration, with symptoms such as pain and agitation, with the need for intensified care, including comfort measures and support for the family. The nurse plays a crucial role in providing care due to the proximity to the person/family. Thus, the starting question of the Learning Project arises: "What is the intervention of the Specialist Nurse in the care of the patient in LDHL?". The course aimed to develop the common competencies of a specialist nurse and specific competencies in Medical-Surgical Nursing in the area of Palliative Care Nursing, as well as those of a Master's degree, and to contribute to improving the quality of nursing care, focusing on the area of care for patients in the Last Days and Hours of Life. This report, which is part of the 1st Master's Degree in Nursing, specializing in Medical-Surgical Nursing, in the area of Palliative Care, reflects the journey made during the first Clinical Teaching in the Community Palliative Care Support Team and during the implementation and evaluation of this Learning Project applied during the Clinical Teaching in the Intra-Hospital Palliative Care Support Team. The project methodology was used in this process. The strategies used were: consulting service documents; carrying out critical reflections on the practice adopted; bibliographical research; a Scoping Review development; observing clinical practice; providing care; consulting nursing records; elaboration of documents to support clinical practice; consulting experts; training/discussion sessions for teams. With this journey, I consider that I have accomplished the goal I set out, and the results obtained are clearly expressed in this report.
