O presente relatório de estágio de natureza profissional integra-se nas atividades realizadas
durante o curso de Mestrado em Enfermagem Médico-Cirúrgica na área de Enfermagem à
Pessoa em Situação Crítica, inserido na formação pós-graduada da Escola Superior de
Enfermagem do Porto. Este relatório foca-se no desenvolvimento de competências comuns e
específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica na área de
Enfermagem à Pessoa em Situação Crítica (PSC), tendo como enfoque principal o tema da
promoção da segurança do doente crítico no transporte intrahospitalar.
O documento insere-se nas Unidades Curriculares Estágio de natureza profissional com relatório
(Módulo I e Módulo II), sendo que, ao longo do mesmo, pretendeu-se desenvolver competências
avançadas através da articulação entre investigação, ensino e resposta às necessidades da
população em situações críticas. O objetivo principal é desenvolver competências comuns e
específicas do enfermeiro especialista em Enfermagem Médico-Cirúrgica, com foco na promoção
da segurança do doente crítico no transporte intrahospitalar. Este foco abrange a sua
preparação e execução rigorosas, considerando eventos adversos e temas recorrentes
identificados na literatura, bem como as recomendações e guidelines relacionadas com a
segurança neste tipo de transporte.
A segurança do doente emerge como um aspeto fundamental para a prática clínica,
particularmente no contexto da PSC, uma vez que é frequentemente associado a instabilidade
hemodinâmica e falência orgânica. A prevenção, deteção e gestão de possíveis complicações
são essenciais para a promoção da segurança do doente crítico neste contexto. Nesse sentido,
destaca-se o papel preponderante do enfermeiro, que, através do desenvolvimento de
competências específicas, assegura a qualidade dos cuidados prestados a estes doentes.
Do ponto de vista estrutural, este trabalho está dividido em três partes principais. A primeira
refere-se à caracterização dos três contextos clínicos; a segunda contempla a apresentação de
um plano de conceção de cuidados, com o objetivo de expor as competências adquiridas no
âmbito do processo de tomada de decisão na pessoa em situação crítica; a terceira foca-se na
reflexão sobre os contributos que as experiências dos vários contextos de estágio tiveram para
o desenvolvimento das competências comuns e das competências específicas.
O desenvolvimento de competências é um processo dinâmico e contínuo, pelo que não termina
neste percurso. No entanto, este, foi um marco importante quer a nível académico quer
profissional, no qual foi possível adquirir um conjunto de ferramentas que promovem a melhoria
da qualidade dos cuidados prestados.
This professional internship report is part of the activities carried out during the Master's course in Medical-Surgical Nursing in the area of Nursing for People in Critical Situations, as part of the postgraduate training program at the Escola Superior de Enfermagem do Porto. This report focuses on the development of common and specific competences of the Specialist Nurse in Medical-Surgical Nursing in the area of Nursing for People in Critical Situations (PCS), with the main focus on promoting the safety of critically ill patients during intra-hospital transportation. The document is part of the Curricular Units Internship of a professional nature with report (Module I and Module II), the aim of which is to develop advanced skills by linking research, teaching and responding to the needs of the population in critical situations. The main objective is to develop common and specific competencies for nurses specializing in Medical-Surgical Nursing, with a focus on promoting the safety of critically ill patients in intra-hospital transport. This focus encompasses rigorous preparation and execution, considering adverse events and recurring themes identified in the literature, as well as recommendations and guidelines related to safety in this type of transportation. Patient safety emerges as a fundamental aspect of clinical practice, particularly in the context of PCS, since it is often associated with hemodynamic instability and organ failure. The prevention, detection and management of possible complications are essential for promoting the safety of critically ill patients in this context. In this sense, the preponderant role of the nurse stands out, who, through the development of specific skills, ensures the quality of care provided to these patients. From a structural point of view, this work is divided into three main parts. The first refers to the characterization of the three clinical contexts; the second includes the presentation of a care design plan, with the aim of explaining the competences acquired in the decision-making process for people in critical situations; the third focuses on reflecting on the contributions that the experiences of the various internship contexts had on the development of common competences and specific competences. Skills development is a dynamic and continuous process, so it doesn't end with this course. However, this was an important milestone, both academically and professionally, in which it was possible to acquire a set of tools to improve the quality of care provided.
