Abstract(s)
A pessoa submetida a cirurgia abdominal major encontra-se em risco de
desenvolver múltiplas complicações relacionadas com capacidade funcional pré operatória, com o tipo e duração da cirurgia e da anestesia, com a idade e presença
de comorbilidades. A construção de uma ostomia de eliminação gera uma situação
de vulnerabilidade acrescida pelo risco de desenvolvimento de hérnia paraestomal.
Estes fatores influenciam fortemente a capacidade da pessoa para o desempenho
das atividades de vida diária, aumentam significativamente o tempo de estadia no
hospital e os custos associados aos cuidados de saúde. O impacto negativo na
qualidade de vida e no desempenho do autocuidado tornam esta temática
importante para a prática clínica dos enfermeiros especialistas em enfermagem de
reabilitação.
Para aprofundar o estudo nesta área foi realizada uma revisão scoping da
temática. Posteriormente foi implementado em contexto de ensino clínico um projeto
de estágio que contemplava o desenvolvimento das competências específicas de
intervenção do enfermeiro especialista em enfermagem de reabilitação. Os objetivos
e as atividades planeadas no projeto de estágio contemplavam também o
desenvolvimento das competências comuns do enfermeiro especialista e as
restantes competências específicas do enfermeiro especialista em enfermagem de
reabilitação definidas pela Ordem dos Enfermeiros em Diário da República.
Os estágios foram realizados em contexto hospitalar e em contexto
comunitário. De forma a responder a todos os objetivos e atividades propostos, no
contexto hospitalar optei por realizar ensino clínico em ambulatório (unidade de
digestivo e unidade de urologia) e numa unidade de cuidados intensivos. No
contexto comunitário, o ensino clínico foi realizado numa equipa de cuidados
continuados integrados.
O presente relatório espelha o percurso realizado e a implementação do
projeto de estágio, bem como demonstra a aquisição e desenvolvimento das
competências referidas.
The person undergoing abdominal surgery is at major risk of developing multiple complications related to pre-operative functional capacity, type and duration of surgery and anaesthesia, age and presence of comorbidities. The construction of an elimination ostomy generates an increment of vulnerability and the risk of developing paraestomal hernia. These factors strongly relate to the person ability to perform his/her activities of daily living, significantly increasing the duration of hospitalization and its associated costs. Also, its negative impact in quality of life and ability to self-care, state the importance of this matter for the clinical practice of Rehabilitation Specialist Nurses. A scoping review of this theme was performed in order to in-depth its study. Afterwards it was implemented in clinical practice as an Internship Project which contemplated the development of specific skills related to the practice of the Rehabilitation Specialist Nurse. The objectives and planned activities of this Internship Project also contemplate the development of general skills, related to the Specialist Nurse Diploma, published and defined by the Portuguese Order of Nurses In Diário da República. The internships occurred in Hospital and Community context. In order to respond to all proposed objectives and activities, in the Hospital Context I have decided to do my internship in two Outpatient Units (Gastrointestinal and Urology) and in Intensive Care Unit. Regarding the Community Context, the internship was performed in Home Health Care environment. The present report outlines the chosen direction and its implementations in the Internship Project, as demonstrates the acquisition and development of referred general and specific skills.
