O fenómeno do turnover em enfermagem é um problema crescente
internacional e nacional, pondo em causa a sustentabilidade dos serviços de saúde,
provocando uma diminuição na Qualidade dos Cuidados de Enfermagem e na
Satisfação dos Enfermeiros, tendo vindo a ser associado a Ambientes da Prática de
Enfermagem desfavoráveis. O Ambiente de Prática de Enfermagem (APE) tem
impacto na Segurança dos doentes, nos resultados de enfermagem, nas equipas e na
eficiência das organizações, promovendo desta forma, a motivação dos profissionais,
diminuição da intenção do turnover e do burnout dos enfermeiros. Os enfermeiros
gestores poderão desenvolver estratégias que contribuam para a retenção dos
enfemeiros através da antecipação da intenção dos enfermeiros em sair, quando esta
se encontra relacionada com fatores do Ambiente de Prática de Enfermagem.
Deste modo, como objetivo geral pretendeu-se analisar a relação entre o
Ambiente de Prática de Enfermagem e a Intenção de Turnover em contexto hospitalar.
Como objetivos específicos pretende-se (1) Avaliar o Ambiente de Prática de
Enfermagem, (2) Avaliar a Intenção de Turnover e (3) Descrever a relação entre as
duas variáveis. Desenvolveu-se um estudo quantitativo, descritivo e correlacional num
hospital da região de Lisboa. Como instrumento de recolha de dados foi aplicado um
questionário que inclui a versão portuguesa da Practice Environment Scale of the
Nursing Work Index (PES-NWI) e a Escala de Intenção de Turnover. A amostra final
foi composta por 155 enfermeiros. Os dados foram analisados com recurso ao
programa Statistical Package for the Social Science (SPSS), versão 25.
Os resultados revelam um Ambiente de Prática de enfermagem misto, com
média de valores de 2,57, sendo que se verificou duas dimensões desfavoráveis,
nomeadamente a “Participação na governação interna nos hospitais” e a “Adequação
dos Recursos Humanos e Materiais”; e uma Intenção de Turnover significativa. Os
resultados revelam uma relação inversa entre o Ambiente de Prática de Enfermagem
e a Intenção de Turnover mas que se verificou de baixa intensidade.
Esta é uma área ainda com necessidade de desenvolvimento em Portugal dado
o número reduzido de estudos. Os enfermeiros gestores devem contribuir para o
desenvolvimento de Ambiente de Prática de Cuidados favoráveis, promovendo a
retenção dos enfermeiros. São necessários mais estudos para compreender os
fatores preditores da intenção de turnover nesta organização.
The phenomenon of Turnover in nursing is an international and national growing problem, jeopardizing the sustainability of health services, causing a decrease in the Quality of Nursing Care and in Job Satisfaction, having been associated with unfavorable Nursing Practice Environments. The Nursing Practice Environment has an impact on patient safety, on nursing and team outcomes, and on the efficiency of organizations, thus promoting motivation, decreasing turnover intention and burnout. Nurse managers develop strategies that contribute to the retention of nurses by anticipating the nurses' intention to leave, when this is related to factors in the Nursing Practice Environment. As a general objective, it is intended to analyze the relationship between the Nursing Practice Environment and a Turnover Intention in a hospital context. The specific objectives intended were (1) Evaluate the Nursing Practice Environment, (2) Evaluate the Turnover Intention and (3) to Describe the relationship between the two variables. A quantitative, descriptive and correlational study was carried out in a hospital in Lisbon. As a data collection instrument, a form was provided that includes the Portuguese version of the Nursing Work Index Practice Environment Scale (PES NWI) and the Anticipated Turnover Scale.
