Abstract(s)
As infeções associadas aos cuidados de saúde (IACS) são eventos adversos que
afetam a pessoa em situação crítica (PSC), especialmente quando internada em unidade
de cuidados intensivos (UCI), sendo a mais prevalente a pneumonia associada à
intubação (PAI), área esta sensível aos cuidados de Enfermagem. Assim, a
implementação de estratégias de prevenção e controlo de infeção, concretamente da
PAI, promovem a prevenção de complicações e a segurança do doente, sendo
fundamental na educação e prática de enfermagem.
O desenvolvimento da temática em estudo, serviu de fio condutor neste percurso
e foi sustentada na Teoria dos Sistemas de Betty Neuman, que considera a pessoa em
constante relação com o meio, sendo a PSC vulnerável pela quebra nas suas linhas de
defesa, e a intervenção de enfermagem crucial na prevenção da PAI, mitigação do seu
impacto e promoção da sua recuperação, de acordo com os três níveis de prevenção
por si preconizados.
Para desenvolver competências de mestre e especialista na área da pessoa em
situação critica, tendo por base o modelo de Dreyfus, adaptado por Benner, foram
realizados estágios em contexto de urgência, unidade de cuidados intensivos e Unidade
local de Programa de Prevenção e Controlo de Infeção e Resistência aos
Antimicrobianos. Para cada um deles, foram traçados objetivos gerais, específicos e
atividades para os concretizar e que permitiram o culminar de um processo de
desenvolvimento de competências de mestre e especialista na área de enfermagem à
PSC, num estádio superior ao do início deste percurso.
Healthcare-associated infections (HAIs) are adverse events that affect critically ill patients, especially when they are hospitalised in an intensive care unit (ICU). The most prevalent of these is intubation-associated pneumonia (IAP), an area that is sensitive to nursing care. Thus, implementing infection prevention and control strategies, specifically for IAP, promotes the prevention of complications and patient safety and is fundamental to nursing education and practice. The theme under study, guided by Betty Neuman's Systems Theory, views the person as constantly interacting with the environment. In the case of critically ill patients, their vulnerability due to the breakdown in their lines of defence underscores the crucial role of nursing intervention in preventing IAP, mitigating its impact, and promoting recovery, in line with Neuman's three levels of prevention. To develop master's and specialist competencies in the critically ill person, based on the Dreyfus model, adapted by Benner, internships were carried out in an emergency room, intensive care unit and local Infection Prevention and Control and Antimicrobial Resistance Programme Unit. For each of them, general and specific objectives were set, as well as activities to achieve them, which led to the culmination of developing master's and specialist competencies in nursing for people in critical situations at a higher stage than at the beginning of this journey.
