Abstract(s)
A transição de cuidados é um momento crucial que ocorre frequentemente nas
Instituições de Saúde, uma vez que permite a passagem de informação de um profissional de
saúde para outro, dando continuidade aos cuidados prestados.
Com o presente relatório pretende-se abordar as diferentes etapas de um Projeto de
Intervenção, cujo objetivo principal foi garantir uma comunicação eficaz na transição de
cuidados de saúde, promovendo a segurança do doente relativamente às últimas orientações
emanadas pela DGS no que diz respeito à Transição de Cuidados, sendo criada uma ferramenta
estruturada a ser usada nos momentos de transição de cuidados dos doentes que são
transferidos do Serviço de Urgência para outras Unidades de Internamento, avaliando-se o seu
cumprimento através da observação direta dos profissionais.
Pretende-se ainda descrever o caminho de aprendizagens percorrido, rumo à aquisição
de competências do Enfermeiro Especialista em pessoa em situação crítica, bem como
competências de Mestre em Enfermagem.
The transition of care is a crucial moment that occurs frequently in Health Institutions, since it allows the passage of information from one health care professional to another, giving continuity to the care provided. This report intends to address the different stages of an Intervention Project, the main objective of which was to ensure effective communication in the transition of health care, promoting patient safety in relation to the latest guidelines emanated from the DGS as regards the Care Transition, and a structured tool is created to be used in the transition of care of patients who are transferred from the Urgency Service to other Internment Units, evaluating their Through direct observation of professionals. It is also intended to describe the path of learning, to acquire the skills of the Specialist Nurse in a critical situation, as well as Master's in Nursing.
