O presente Relatório de Estágio, visa explanar através do exercício critico-reflexivo as atividades
desenvolvidas e estratégias implementadas ao longo de Estágio I e Estágio Final. Pretende,
também, ser um contributo para o desenvolvimento e aquisição das competências comuns e
específicas do enfermeiro especialista em Enfermagem Médico-Cirúrgica: A Pessoa em Situação
Crítica e de Mestre em Enfermagem. As pessoas internadas em unidades de cuidados intensivos
predispõem de risco elevado de infeção, sendo que o cateter venoso central, é um dispositivo
clínico responsável, por alguma desta casuística. A lesão renal aguda, em contexto de disfunção
multiorgânica, justifica a necessidade de recorrer-se à hemodiálise, através do cateter venoso
central. No entanto este dispositivo apresenta riscos, nomeadamente, elevadas taxas de infeção.
A intervenção especializada do Enfermeiro é fundamental para a qualidade e segurança das
pessoas com este tipo de disposto, em situação de lesão renal aguda. Neste âmbito, foi
desenvolvido um Projeto de Intervenção Major, sustentado pela metodologia de trabalho de
projeto e assente no Modelo Teórico de June Larrabee, Modelo para a Mudança da Prática
Baseada em Evidência. O Projeto teve como intuito a implementação de intervenções de
enfermagem num contexto de Medicina Intensiva, que objetivava contribuir para a melhoria da
qualidade dos cuidados de enfermagem prestados.
This Internship Report aims to explain, through a critical-reflective exercise, the activities developed and strategies implemented throughout Internship I and Final Internship. It also aims to contribute to the development and acquisition of the common and specific skills of the nurse specialist in Medical-Surgical Nursing: The Person in Critical Condition and Master in Nursing. People admitted to intensive care units are at high risk of infection, and the central venous catheter is a clinical device responsible for some of these cases. A lesão renal aguda, em contexto de disfunção multiorgânica, justifica a necessidade de recorrer-se à hemodiálise, através do cateter venoso central. However, this device presents risks, namely high rates of infection. The specialised intervention of the Nurse is essential for the quality and safety of people with this type of provision, in a situation of acute kidney injury. In this context, a Major Intervention Project was developed, supported by the project work methodology and based on June Larrabee's Theoretical Model for Change in Evidence-Based Practice. The project aimed at implementing nursing interventions in an Intensive Care Medicine setting, with the purpose of contributing to improving the quality of nursing care.
