O presente relatório surge no âmbito da Unidade Curricular “Estágio de Natureza Profissional
com Relatório – Módulo II” e pretende explanar o processo de desenvolvimento das
competências comuns e das competências específicas do Enfermeiro Especialista em
Enfermagem de Reabilitação, ao longo do estágio que decorreu em diferentes contextos clínicos
da zona norte do país, em contexto hospitalar, nomeadamente nos serviços de neurocirurgia,
ortopedia, pneumologia, e em contexto comunitário, na unidade de cuidados na comunidade e
na unidade de reabilitação e manutenção.
O Enfermeiro Especialista em Enfermagem de Reabilitação possui um papel fundamental no
acompanhamento das pessoas que estão a vivenciar processos de transição, implementando
intervenções tendo em vista a promoção do autocontrolo e capacitação para o autocuidado. A
implementação de intervenções com vista à promoção da autonomia e recuperação da
funcionalidade da pessoa, além de melhorarem a função, são determinantes para o processo
adaptativo e, consequentemente, para a qualidade de vida da pessoa alvo dos cuidados.
A par da apresentação dos contextos onde se realizou o estágio e da reflexão sobre o
desenvolvimento das competências comuns e específicas do Enfermeiro Especialista em
Enfermagem de Reabilitação, deste relatório consta ainda a explanação de dois casos clínicos,
um da vertente neurológica - pessoa que sofreu um traumatismo crânio encefálico, e um
segundo caso da componente ortotraumatológica – pessoa com fratura do colo do fémur na
sequência de uma queda. Em cada um dos casos foram identificados os domínios, os dados
relevantes, os diagnósticos no âmbito dos processos corporais e dos processos adaptativos,
assim como definidos os objetivos, planeadas as intervenções e, consequentemente, avaliados
os resultados das intervenções implementadas. A escolha dos casos prendeu-se com o facto das
necessidades das doentes no âmbito da cognição, permitirem o desenvolvimento de
competências especializadas para a tomada de decisão clínica de enfermagem de reabilitação
no domínio da cognição.
No sentido de capacitar a pessoa para realizar as atividades do dia a dia, é necessário que esta
tenha a capacidade para aprender nova informação e reaprender as atividades inerentes à
concretização do autocuidado. Assim, e perante a identificação dos dados relevantes através da
utilização das escalas de avaliação, da avaliação do potencial para melhorar a cognição e
avaliação do conhecimento e capacidade para a realização das atividades que promovam a
cognição, foi possível incluir na conceção de cuidados, intervenções centradas na recuperação
da função cognitiva. Atendendo à evidência relativamente aos benefícios da estimulação cognitiva, e à necessidade
de sistematizar as intervenções do Enfermeiro Especialista em Enfermagem de Reabilitação, foi
desenvolvido um programa de exercícios de estimulação cognitiva centrados nas atividades que
concretizam os autocuidados. Os exercícios estão direcionados para as diferentes funções
cognitivas e focados nos autocuidados Tomar Banho, Vestir-se/Despir-se, Uso do sanitário e
Alimentar-se.
This report emerges within the scope of the Curricular Unit "Professional Nature Internship with Report - Module II" and aims to explain the process of developing common competencies and specific competencies of the Nurse Specialist in Rehabilitation Nursing, throughout the internship that took place in different clinical settings in the northern region of the country, in a hospital context, namely in neurosurgery, orthopedics, pneumology, and in a community context, in the community care unit and in the rehabilitation and maintenance unit. The nurse specialist in rehabilitation nursing plays a fundamental role in accompanying individuals experiencing transition processes, implementing interventions aimed at promoting autonomy and recovery of the individual's functionality, besides improving function, are crucial for the adaptive process and consequently for the quality of life of the person receiving care. In addition to presenting the contexts in which the internship took place and reflecting on the development of common and specific competencies oh the nurse specialist in rehabilitation nursing, this report also includes the explanation of two clinical cases, one neurological- a person who suffered a traumatic brain injury, and a second case of orthopedic trauma- a person with a femoral neck fracture following a fall. In each case, the domains relevant data, diagnoses within the scope of bodily processes and adaptive processes were identified, as well as objectives defined, interventions planned and consequently the results of the implemented interventions evaluated. The choice of cases was due to the fact that the patients needs in terms of cognition allowed for the development of specialized competencies for clinical nursing decision making in the domain of cognition. In order to empower the individual to perform activities of daily living, it is necessary for them to have the ability to learn new information and relearn the activities inherent in self care. Thus, and in the face of identifying relevant data through the use of assessment scales, assessing the potential for improving cognition and asessing knowledge and capacity for performing activities that promote cognition, it was possible to include in the care conception, interventions focused on the recovery of cognitive function. Given the evidence regarding the benefits of cognitive stimulation and the need to systematized interventions by the nurse specialist in rehabilitation nursing, a program of cognitive stimulation exercises centered on activities that realize self care was developed. The exercises are directed towards different cognitive functions and focused on self care activities such as bathing, dressing/undressing, toilet use and feeding.
