As doenças crónicas são um dos principais desafios do século XXI. Entendidas como doenças de
longa duração e, na sua maioria, de progressão lenta, exigem cuidados e tratamentos contínuos
ao longo do tempo, destacando-se a infeção por VIH.
O presente relatório foi elaborado no âmbito do estágio de natureza profissional, módulo II,
inserido no plano de estudos do Mestrado em Enfermagem Médico-Cirúrgica na área da Pessoa
em Situação Crónica, da Escola Superior de Enfermagem do Porto. Este documento reflete o
pensamento descritivo e crítico-reflexivo sobre o desenvolvimento das competências comuns e
específicas, inerentes à especialização em Enfermagem nesta área.
No contexto deste estágio, foi desenvolvido inicialmente um projeto de âmbito académico
intitulado “Gestão da Doença crónica na pessoa com infeção VIH”, com o objetivo de auxiliar no
desenvolvimento de competências em Enfermagem Médico-Cirúrgica, na área de Enfermagem à
Pessoa em Situação Crónica, mais especificamente no âmbito do cuidado à pessoa que vive com
VIH. Este projeto foi implementado no estágio de natureza profissional realizado em contexto de
internamento de Doenças Infeciosas e na Consulta externa da mesma especialidade, num
Serviço de Ambulatório, na área da Consulta de enfermagem de Doenças Infeciosas. Tendo por
base as competências definidas pela Ordem dos Enfermeiros, foram estabelecidos objetivos
específicos com orientação para os resultados pretendidos, bem como atividades que
permitiram a sua operacionalização.
Como forma de demonstração do impacto do papel do enfermeiro especialista, bem como da
atividade e dos cuidados de enfermagem, direcionados a estas pessoas, foi incluído neste
relatório a conceção de cuidados a um caso clínico experienciado, na área da consulta externa.
Na explanação do processo de tomada de decisão especializada, relacionada com a vivência da
doença crónica na pessoa com Infeção VIH, recorreu-se ao aplicativo educacional e4nursing, que
integra a ontologia de enfermagem. Norteados pelo propósito de promover uma transição fluida
e de capacitar a pessoa que vive com infeção VIH para a autogestão da doença e do regime
terapêutico, foram implementados cuidados de enfermagem especializados afetos a este
contexto, orientados às respostas humanas ao processo de transição da pessoa que vive com
VIH, através da definição de estratégias de intervenção adequadas e eficazes.
Chronic diseases are one of the main challenges of the 21st century. For the most part, understood as long-term diseases and slow progression, which gives rise to the need of continued care and treatment over time, as HIV infection. This report was prepared within the scope of the professional internship, module II, included in the study plan of the Master's Degree in Medical-Surgical Nursing in the area of People in Chronic Situations, at the Escola Superior de Enfermagem do Porto. This document reflects descriptive and critical-reflective thinking on the development of common and specific skills inherent to specialization in Nursing in this area. In the context of this internship, an academic project entitled “Management of Chronic Disease in People with HIV Infection” was initially developed, with the aim of assisting in the development of skills in Medical-Surgical Nursing, in the area of Nursing for People in Chronic Situations, more specifically in the scope of care for people living with HIV. This project was implemented in the professional internship carried out in the context of hospitalization for Infectious Diseases and in the external consultation of the same specialty, in Outpatient Service, in the area of Nursing Consultation for Infectious Diseases. Based on the competencies defined by the Order of Nurses, specific objectives were established with guidance towards the intended results, as well as activities that allowed their operationalization. As a way of demonstrating the impact of the role of the specialist nurse, as well as the nursing activity and care directed at these people, the design of care for a clinical case experienced in the area of outpatient consultation was included in this report. In the explanation of the specialized decision-making process, related to the experience of chronic illness in people with HIV infection, the educational application e4nursing, which integrates the nursing ontology, was used. Guided by the purpose of promoting a fluid transition and empowering people living with HIV infection to self-manage the disease and the therapeutic regimen, specialized nursing care was implemented for this context, oriented towards human responses to the transition process of people living with HIV, through the definition of appropriate and effective intervention strategies.
