Este documento traduz um resumo de todo o trabalho clínico desenvolvido no âmbito do Estágio
de natureza profissional com relatório final, integrante no Curso de Mestrado em Enfermagem
Médico-Cirúrgica, na área de Enfermagem à Pessoa em Situação Paliativa. O interesse em
abordar a Oclusão Intestinal Maligna em doentes em contexto de Cuidados Paliativos, surge
devido ao facto de, na prática diária de enfermagem depararmo-nos com dificuldades em gerir
todos os sintomas que este diagnóstico provoca. Teve por base a necessidade de aprofundar
conhecimentos sobre a forma como os sintomas surgem, de como a pessoa encara e vive os
sintomas, que muitas vezes surgem em simultâneo. É importante, nestas pessoas, ter em conta
as suas necessidades, vontades e preferências, adequando o plano de cuidados a todos a esses
níveis.
O objetivo foi desenvolver competências comuns e especializadas em Enfermagem Médico-
Cirúrgica, na área de Enfermagem à Pessoa em Situação Paliativa e identificar intervenções de
conforto para doentes com diagnóstico de Oclusão Intestinal Maligna.
O método utilizado foi baseado na orientação para as atividades que concretizam o perfil de
competências explanadas pelo regulamento nº 429/2018 e regulamento nº 140/2019 da Ordem
dos Enfermeiros, desenvolvimento de competências e crítica reflexiva apoiada na revisão de
literatura e pesquisa da melhor evidência científica disponível.
Os resultados expõem–se via dois casos clínicos desenvolvidos na plataforma E4Nursing da
Escola Superior de Enfermagem do Porto, bem como, a realização de uma formação em serviço
sobre Intervenções de Conforto para o Controlo sintomático da Oclusão Intestinal Maligna. O
foco de desenvolvimento das competências clínicas vai reforçar a área do conforto em Cuidados
Paliativos, nomeadamente no controlo sintomático da dor abdominal, náuseas, vómitos e o
efeito que o descontrolo sintomático pode trazer aos doentes com Oclusão Intestinal Maligna do
ponto de vista físico, emocional e espiritual.
Em cuidados paliativos, os cuidados de enfermagem da pessoa com Oclusão Intestinal Maligna,
têm como objetivo, o controlo sintomático de forma a promover o conforto, a dignidade da
pessoa e da sua família. Os sintomas que ocorrem perante este diagnóstico são efetivamente de
difícil controlo, porém existem intervenções de enfermagem que contribuem para minimizar o
sofrimento em fim de vida
This document is a summary of all the clinical work carried out as part of the professional internship with final report, which is part of the Master's Degree in Medical-Surgical Nursing, in the area of Palliative Care Nursing. The interest in approaching Malignant Intestinal Occlusion in Palliative Care patients arises from the fact that, in daily nursing practice, we find it difficult to manage all the symptoms that this diagnosis causes. It was based on the need to deepen our knowledge of how the symptoms appear, how the person faces and experiences the symptoms, which often appear simultaneously. It is important for these people to take into account their needs, wishes and preferences, adapting the care plan to all of these levels. The aim was to develop common and specialized skills in Medical-Surgical Nursing, in the area of Palliative Care Nursing, and to identify comfort interventions for patients diagnosed with Malignant Intestinal Occlusion. The method used was based on orientation towards the activities that materialize the competency profile explained by regulation nº 429/2018 and regulation nº 140/2019 of the Order of Nurses, competency development and reflective criticism supported by a literature review and search for the best available scientific evidence. The results are presented via two clinical cases developed on the E4Nursing platform of the Escola Superior de Enfermagem do Porto, as well as in-service training on Comfort Interventions for the Symptomatic Control of Malignant Intestinal Occlusion. The focus for developing clinical skills will be on comfort in Palliative Care, namely symptomatic control of abdominal pain, nausea, vomiting and the effect that symptomatic uncontrol can have on patients with Malignant Intestinal Occlusion from a physical, emotional and spiritual point of view. In palliative care, nursing care for people with Malignant Intestinal Occlusion, is aimed, at symptomatic control in order to promote the comfort and dignity of the person and their family. The symptoms that occur with this diagnosis are indeed difficult to control, but there are nursing interventions that help to minimize suffering at the end of life.
