dc.description.abstractO Pé Diabético é uma das complicações graves mais comuns na pessoa com diabetes. A identificação e prevenção do Pé Diabético previne a maioria das amputações, o impacto socioeconómico deste e aumenta a qualidade de vida da pessoa com diabetes e respetiva família. O Projeto de Intervenção Comunitária proporcionou a construção, implementação e avaliação de um projeto útil e exequível na área do Pé Diabético. Baseados nas necessidades identificadas pela e com a equipa de enfermagem, pretendeu-se promover as atividades de identificação e prevenção do Pé Diabético, através da Estruturação da Consulta de Enfermagem do Pé Diabético. Adotou-se a metodologia de Planeamento em Saúde e implementaram-se intervenções que envolveram a equipa de enfermagem, promovendo a discussão fundamentada na reflexão, contribuindo para a sua motivação e para o trabalho de parceria. A avaliação do Projeto permitiu concluir que os objetivos foram atingidos e o Projeto enriqueceu a oferta de serviços da UCSPpt_PT
dc.description.abstractDiabetic foot is one of the most common serious complications in people with diabetes. The identification and prevention of Diabetic Foot prevents most amputations, the socioeconomic impact of it and increases the quality of life of people with diabetes and their families. The Community Intervention Project allowed the construction, implementation and evaluation of a useful and feasible project in the area of Diabetic Foot. Based on the needs identified by and with the nursing team, we intended to promote the identification and prevention activities of the Diabetic Foot, by Structuring the Diabetic Foot Nursing Consultation. The Health Planning methodology was adopted and interventions were implemented that involved the nursing team, promoting discussion based on reflection, contributing to their motivation and partnership work. The evaluation of the Project led to the conclusion that the objectives were achieved and the Project enriched the UCSP service offerpt_PT
