Abstract(s)
O sofrimento das pessoas de quem cuido, numa equipa dos cuidados de saúde
primários dedicada àquelas em situação de vulnerabilidade acrescida foi o ponto de
partida para o meu percurso enquanto Enfermeira Especialista em Saúde Mental e
Psiquiatria (EESMP).
Este relatório pretende espelhar as aprendizagens ocorridas durante o estágio
cujos objetivos foram: desenvolver competências pessoais e profissionais no âmbito
da gestão emocional da pessoa com experiência de doença mental e desenvolver
um projeto de intervenção de promoção da capacitação da pessoa com experiência
de doença mental na gestão das emoções. O estágio decorreu de 23 setembro 2019
a 07 de fevereiro de 2020 (nove semanas numa Equipa Comunitária - EC-A - e nove
em internamento hospitalar - IP), num serviço de saúde mental da área
metropolitana de Lisboa.
Foi utilizada uma estratégia baseada na metodologia qualitativa. Através da
prestação de cuidados, recorrendo à observação e à realização de entrevistas não
estruturadas e à recolha de dados através de notas de campo. Valoriza-se as
narrativas e os significados de vida atribuídos pelas pessoas às suas vivências e
experiências de doença mental.
Na EC-A foi privilegiada a relação dual, onde foram estabelecidos planos de
cuidados individualizados e em parceria com alguns utentes, sendo realizadas
entrevistas não estruturadas aos utentes e profissionais da EC-A e consultado o
processo clínico. No IP, foi delineado um conjunto de intervenções psicoterapêuticas
de grupo, com o objetivo de ajudar a PEDM a gerir as emoções. Foram realizadas
quatro sessões com recurso a mediadores expressivos, cada uma com duração
aproximada de 60 minutos. Foi selecionado um grupo de pessoas que se
encontravam internadas no IP com alterações emocionais. As narrativas às obras
criadas foram o ponto de partida para a ressignificação dos acontecimentos de vida.
O percurso relatado, pautado pela partilha de experiências com pessoas, que
são mais do que a sua doença, permitiu concluir que: os processos de recovery são
passíveis de serem promovidos em todos os contextos, a todas as pessoas; a
utilização de mediadores expressivos permitiu a expressão das emoções, potenciou
o autoconhecimento e melhorou o bem-estar; as particularidades de cada pessoa,
(gostos, aspirações, projetos, emoções e sentimentos), podem traduzir-se nas suas
forças; o recurso à emotividade no cuidar é potenciador de resultados benéficos
para as pessoas e para o EESMP.
The suffering of those who I take care of, in a community-based unit dedicated to people with vulnerabilities, was the beginning of my journey as mental health and psychiatry specialist nurse. The purpose of this report is to reflect the learnings developed during the internship. The objectives for this internship were: to develop personal and professional competencies for the emotional management of people that experience mental illness; to develop an intervention project that promotes the empowerment of the people that experience mental illness on emotional management. The internship took place between 23rdSeptember 2019 and 7thFebruary 2020, on a mental health service of Lisbon’s metropolitan area (nine weeks in a community-based team and other nine weeks in a psychiatric ward). The strategy that was used, was based on qualitative methodology: by providing nursing care, employing observation, performing unstructured interviews and collecting data from field notes. It was valued the narrative and meanings to their life, attributed by people that suffer from mental illness, to their own experiences and perceived feelings with mental disease. At the community-based team, a dual relationship was privileged, by establishing individualized care plans in partnership with some patients, also by conducting unstructured interviews with the users and health care professionals and consulting clinical records. In the psychiatric ward, was established a set of psychotherapeutic group interventions, with the goal of helping people with mental illness managing their emotions. Four sessions of psychotherapeutical interventions were made, each with approximate duration of 60 minutes, using expressive mediators. A group of hospitalized patients with emotional distress was choose. The narratives to the created works was the beginning of new meanings for their life events. The reported path, based on shared experiences with people, who are more than the illness they live with, lead me to conclude that: recovery processes are likely to be promoted in all contexts, for all people; the use of expressive mediators allowed the expression of emotions, boosted self-knowledge and improved well-being; each person characteristics (personal taste, aspirations, projects, emotions and feelings), can be revealed in their strengths; the resource to emotional care is an enhancer of beneficial results for people and the mental health and psychiatry specialist nurse.
