Abstract(s)
As alterações
This report was carried out within the scope of the curricular unit Estágio de natureza profissional com relatório - Módulo II, in the academic year 2023/2024, of the Master's Degree in Community Nursing in the area of Family Health Nursing at the Escola Superior de Enfermagem do Porto. The main objective of this report is to demonstrate the acquisition of skills in the speciality of Family Health Nursing. The relevance of Primary Health Care, as proximity care and citizens' gateway to health care, has never been as relevant as it is today. Demographic changes with the aging of the population and the consequent increase of chronic diseases put the sustainability of health systems at risk. To this extent, the growing development of Family Health Nursing in Primary Health Care is increasingly necessary as a way of responding to the needs of the population. While action focused on the promotion and protection of health is desirable, it is not possible to ignore that this response often involves promoting self-management of chronic disease and preventing its complications. Community nurses have the opportunity to get to know each person and their context well. The family that experiences the chronic illness of one of its members is part of this response as a force, but also with needs arising from the changes that its normal functioning and dynamics may undergo. The current paradigm of care points towards promoting empowerment, that is providing users and their families with tools to experience the transitions arising from the diagnosis of a chronic disease, such as Diabetes, as fully and comprehensively as possible. This report shows that each family responds differently to the diagnosis of Diabetes in one of its members. The most relevant aspect in this entire process, more than what the nurse specialist in family health nursing intervention is, is the response of each family to that same interaction.
