O presente relatório de estágio surge no âmbito do término do curso de Mestrado em
Enfermagem Médico-Cirúrgica, na área da Enfermagem à Pessoa em Situação Crítica, lecionado
na Escola Superior de Enfermagem do Porto. Este documento incorpora o que emergiu das
Unidades Curriculares “Estágio de natureza profissional com relatório, Módulo I e II”,
descrevendo de forma sistematizada as atividades e reflexões conducentes à aquisição de
competências, comuns e específicas, do Enfermeiro Especialista na área de Enfermagem à
Pessoa em Situação Crítica, tal como preconizado nos Regulamentos nº140/2019 e nº429/2018
da Ordem dos Enfermeiros.
O estágio de natureza profissional teve como principal objetivo o desenvolvimento de
competências de Enfermagem médico-cirúrgica, na área da enfermagem à pessoa em situação
crítica, com particular enfoque na pessoa em processo de desabituação da ventilação mecânica
invasiva para a ventilação espontânea. Neste sentido foi delineado um projeto de
desenvolvimento de competências com a temática “Processo de desabituação da Ventilação
Mecânica Invasiva para a Ventilação Espontânea”, que serviu como um guia orientador do
estágio, tendo sido implementado e consolidado nos três contextos clínicos onde decorreu o
percurso formativo (Unidade de Cuidados Intensivos Polivalente, Unidade de Cuidados
Intensivos Cardiotorácica e Serviço de Urgência).
O suporte ventilatório com recurso à ventilação mecânica invasiva é um pilar fundamental no
tratamento da pessoa em situação crítica, contudo está associado a múltiplas complicações,
pelo que se preconiza um processo de desabituação da ventilação mecânica invasiva para a
ventilação espontânea oportuno, sustentado em conhecimentos e aptidões, no sentido de
fornecer cuidados seguros e de qualidade.
Estruturalmente, o presente documento está dividido em três partes: caracterização dos
contextos onde decorreu a prática clínica, experiências proporcionadas e sua adequação para a
formação de enfermeiros especialistas; apresentação de três casos com o objetivo de revelar
como o processo de conceção de cuidados foi realizado para todos os doentes a quem prestei
cuidados durante os estágios; reflexão sobre o meu processo de desenvolvimento das
competências comuns e especificas do Enfermeiro Especialista na área de Enfermagem à
Pessoa em Situação Crítica. A elaboração deste relatório foi sustentada na plataforma
pedagógica da Escola Superior de Enfermagem do Porto, "e4nursing", em vigor na instituição.
A complexidade inerente aos cuidados à pessoa em situação crítica exigem cada vez mais, uma
prática especializada que assente nas competências preconizadas para o enfermeiro especialista e mestre em Enfermagem Médico-Cirúrgica. A aprendizagem no contexto de estágio
passa não só pela organização de conceitos teóricos e práticos, mas pela concetualização da
ação refletida e modificada baseada na evidência científica. Todo este processo culminou na
mobilização de saberes especializados que servirão de orientação para uma prática profissional
consciente, segura, com responsabilidade ética e deontológica, visando a qualidade do cuidar
em enfermagem.
This internship report is a component of the Master’s Degree program in Medical-Surgical Nursing, in the area of Nursing to the Critically ill Patient, offered at the Escola Superior de Enfermagem do Porto. This document encompasses the outcomes of the "Clinical Placement Report, Modules I and II," systematically describing the activities and reflections leading to the acquisition of common and specific skills of the Specialist Nurse in the area of Nursing to the Critically Ill Patients, as outlined in Regulations no. 140/2019 and no. 429/2018 of the Ordem dos Enfermeiros. The primary objective of the Clinical Placement was to develop medical-surgical nursing skills in the area of Nursing to the Critically Ill Patients, with a particular focus on individuals undergoing weaning from invasive mechanical ventilation to spontaneous ventilation. To achieve this, a skills development project titled "Process of Weaning from Invasive Mechanical Ventilation to Spontaneous Ventilation" was formulated and served as a guiding framework throughout the internship. This project was implemented and reinforced in three clinical settings where the training took place: the Multipurpose Intensive Care Unit, Cardiothoracic Intensive Care Unit, and Emergency Service. Although ventilatory support through Invasive Mechanical Ventilation is a crucial component in treating critically ill patients, it is associated with multiple complications. Therefore, weaning process from invasive mechanical ventilation to timely spontaneous ventilation is recommended, supported by knowledge and skills, to ensure the delivery of safe and highquality care. Structurally, this document is divided into three parts: characterization of the clinical settings where the practical training occurred, experiences provided and their relevance for the training of specialist nurses; presentation of three study cases to illustrate the care design process for all patients under my care during the internships; reflection on my development of common and specific skills of the Nurse Specialist in Medical-Surgical Nursing, in the area of Nursing to the Critically Ill Patients. The preparation of this report was facilitated by the pedagogical platform of the Escola Superior de Enfermagem do Porto, "e4nursing," utilized by the institution. The increasing complexity in caring for critically ill patients requires specialized practice aligned with the competencies recommended for specialist nurses and master’s in medical-surgical nursing. Learning in the internship context involves not only the integration of theoretical and practical concepts but also the conceptualization of action informed by reflection and scientific evidence. This entire process culminated in the mobilization of specialized knowledge to guide conscientious, safe professional practice, characterized by ethical and deontological responsibility, and aiming for the highest quality of nursing care.
