Introdução: a enfermagem especializada desempenha um papel fundamental nas equipas de saúde paliativas, atendendo à crescente procura de cuidados paliativos, impulsionada não apenas pelo envelhecimento da população, mas também pela crescente prevalência de doenças graves, progressivas e limitantes de vida, que incorrem num intenso sofrimento para clientes e familiares nos aspetos físico, emocional, espiritual e social. Este documento aborda cuidados paliativos e introduz um relatório de Estágio de Natureza Profissional relativo ao 2º curso de Mestrado em Enfermagem Médico-Cirúrgica, na área de Enfermagem à pessoa em situação paliativa da Escola Superior de Enfermagem do Porto. O seu objetivo primordial é descrever de forma refletida as atividades de estágio desenvolvidas, com forte impacto no processo de desenvolvimento das competências comuns e específicas do enfermeiro especialista, em dois de três contextos clínicos de cuidados paliativos especializados, conforme plano de estudos do curso e Regulamentação da Ordem dos Enfermeiros para atribuição do título de especialista. Metodologia: descritiva e reflexiva recorre à melhor evidência científica disponível para justificar processos, percursos e intervenções. Apresentam-se dois estudos de caso com base no modelo E4Nursing trabalhado na Escola Superior de Enfermagem do Porto, no sentido de expor a capacidade reflexiva e raciocínio para a tomada de decisão de enfermagem especializada. Resultados: o presente relatório descreve as caraterísticas dos contextos clínicos onde foram realizados os estágios, apresenta uma revisão de literatura com objetivo de caraterizar o conceito de dor total e intervenção na dor para alívio do sofrimento, privilegiando uma visão holística do cliente. Apresenta uma narrativa reflexiva sobre o desenvolvimento das competências comuns e específicas de acordo com os Regulamentos nº 429 de 2018 de 16 de julho e o nº 140 de 2019 de 6 de fevereiro, publicados em Diário da República. Em síntese, o relatório de estágio promoveu uma reflexão sobre o desenvolvimento de um perfil de competências clínicas que caraterizam a área profissional especializada, assegurando a prestação de cuidados adequados às necessidades multidimensionais dos clientes em situação paliativa e cuidadores/familiares.

This document addresses palliative care and introduces a Professional Internship report relating to the 2nd Master's course in Medical-Surgical Nursing, in the area of Nursing for people in palliative situations at the Nursing School of Porto. Its primary objective is to describe in a reflective way the internship activities developed, with a strong impact on the process of developing the common and specific skills of specialist nurses, in two of three clinical contexts of specialized palliative care, according to the course study plan and to the Regulation of the Order of Nurses to award the title of Specialist. Methodology: descriptive and reflective, using the best available scientific evidence to justify processes, pathways and interventions. Two case studies are presented based on the E4Nursing model worked at the Nursing School of Porto in order to expose the reflective capacity and decision-making reasoning of specialized nursing. Results: this report describes the characteristics of the clinical contexts where the internships were carried out, presents a literature review with the aim of characterizing the concept of total pain and pain intervention to alleviate suffering, privileging a holistic view of the client. It presents a reflective narrative on the development of common and specific skills in accordance with Regulations no. 429 of July 16th, 2018 and no. 140 of February 6th, 2019 published in Portugal’s Diário da República. In summary, the internship report promoted reflection on the development of a clinical skills profile that characterizes a specialized professional area, ensuring the provision of care appropriate to the multidimensional needs of clients in palliative situations and caregivers/family members.
