Os cuidados de enfermagem prestados à pessoa em situação crítica demandam uma elevada especialização, consequentemente os enfermeiros especialistas em Enfermagem Médico-cirúrgica na Área de Enfermagem à Pessoa em Situação Crítica, são essenciais na resposta às complexas necessidades que emergem em contextos de instabilidade clínica. O transporte intra-hospitalar da pessoa em situação crítica é uma prática recorrente e suscetível à ocorrência de eventos adversos, não só pelo transporte em si, mas pela vulnerabilidade que acresce à pessoa.
Compete ao enfermeiro especialista o planeamento, implementação e avaliação de estratégias para garantir a segurança da pessoa em situação crítica, fundamentando a sua prática em evidências científicas, protocolos institucionais e diretrizes, contribuindo desta forma para a redução dos riscos associados e para a promoção da qualidade e segurança dos cuidados no transporte intra-hospitalar.
Visando investigar e identificar as evidências científicas atuais acerca desde problemática, elaboramos uma scoping review, com base na metodologia de Arksey e O’Malley.
Para a elaboração do presente relatório adotámos por uma abordagem metodológica descritiva, reflexiva e crítica centrada nas experiências vivenciadas ao longo do percurso formativo, evidenciando o desenvolvimento de competências, essenciais à obtenção do título de Enfermeiro Especialista em Enfermagem Médico-Cirúrgica na Área de Enfermagem à Pessoa em Situação Crítica e Mestre em Enfermagem. O estágio foi realizado numa Unidade de Cuidados Intensivos, numa Unidade Local do Programa de Prevenção e Controlo de Infeções e Resistência a Antimicrobianos e numa Viatura Médica de Emergência e Reanimação.
Do ponto de vista concetual, o percurso formativo foi sustentado pelo referencial teórico da Teoria da Competência Tecnológica como Cuidar em Enfermagem de Rozzano Locsin e suportado pelos Regulamentos de Competências Comuns do Enfermeiro Especialista, Competências Específicas do Enfermeiro Especialista em Enfermagem à Pessoa em Situação Crítica e às Competências de Grau de Mestre em Enfermagem, segundo os descritores de Dublin.
Nursing care provided to critically ill patients demands a high level of specialization, consequently, specialist nurses in Medical-Surgical Nursing, particularly in the area of Critical Care Nursing, are essential in responding to the complex needs that arise in contexts of clinical instability. Intra-hospital transport of critically ill patients is a frequent practice and susceptible to adverse events, not only due to the transport itself but also because of the patient’s vulnerability. It is the responsibility of the specialist nurse to plan, implement, and evaluate strategies to ensure the safety of the critically ill patient, grounding their practice in scientific evidence, institutional protocols, and guidelines. In doing so, they contribute to reducing associated risks and promoting the quality and safety of care during intra-hospital transport. To investigate and identify current scientific evidence regarding this issue, we conducted a scoping review based on the Arksey and O’Malley methodology. For the preparation of this report, we adopted a descriptive, reflective, and critical methodological approach centered on the experiences lived throughout the training process, highlighting the development of competencies essential to obtaining the title of Specialist Nurse in Medical-Surgical Nursing in the area of Critical Care Nursing, as well as a Master’s degree in Nursing. The internship was completed in an Intensive Care Unit, a Local Unit of the Program for Prevention and Control of Infections and Antimicrobial Resistance, and an Emergency Medical and Resuscitation Vehicle. From a conceptual standpoint, the training pathway was supported by the theoretical framework of Rozzano Locsin’s Technological Competency as Caring in Nursing Theory, and grounded in the Common Competency Regulations for Specialist Nurses, the Specific Competencies of the Specialist Nurse in Critical Care Nursing, and the Master’s Degree Competencies in Nursing, according to the Dublin descriptors.
