Abstract(s)
A realização deste relatório surge no âmbito da unidade curricular Relatório de Estágio, do 1º Curso de Mestrado em Enfermagem Médico-Cirúrgica, da Escola Superior de Enfermagem São José de Cluny, no referente ano letivo 2015-2017 e a sua apresentação e discussão pública visa a conclusão do 2º ciclo do ensino superior conducente ao grau de mestre.
Com o tema, Competências de Enfermagem Especializadas na Abordagem à Pessoa em Situação Crítica no Pré e Intra Hospitalar, este trabalho descreve todo o percurso realizado, que vai desde a prática clínica em contexto de serviço de urgência e cuidados intensivos, até a prática de opção no pré-hospitalar e incide nas competências comuns e nas específicas do enfermeiro especialista em enfermagem médico-cirúrgica.
Considerando que o saber profissional de enfermagem resulta de um processo de construção, assente numa prática implicada e refletida, o objetivo norteador é evidenciar a mestria em enfermagem médico-cirúrgica, através da descrição, análise e reflexão das intervenções desempenhadas e competências desenvolvidas durante as práticas clínicas, realizando a confrontação do conhecimento científico com as situações práticas experienciadas.
Estas práticas foram desenvolvidas no Serviço de Urgência e no Serviço de Cuidados Intensivos Polivalente do SESARAM, E.P.E e no SEMER que integra a EMIR, do Serviço Regional de Proteção Civil, IP- RAM. O primeiro e último no Estágio I e o segundo no Estágio II respetivamente. Por sua vez o Estágio III – Opção decorreu no Instituto Nacional de Emergência Médica na Delegação Sul, em Lisboa, de modo a desenvolver competências no âmbito pré-hospitalar, uma vez que a pessoa em situação crítica muitas vezes desenvolve o seu processo de doença em ambiente extra-hospitalar.
De forma conclusiva, desenvolvi as competências comuns do enfermeiro especialista e as seguintes competências específicas especializadas: “Cuida da pessoa a vivenciar processos complexos de doença crítica e/ou falência orgânica”, “Dinamiza a resposta a situações de catástrofe ou emergência multi-vítima, da concepção à acção”; “Maximiza a intervenção na prevenção e controlo da infecção perante a pessoa em situação crítica e/ou falência orgânica”; “Cuida de pessoas com doença crónica, incapacitante e terminal, dos seus cuidadores e familiares” e “Estabelece relação terapêutica com pessoas com doença crónica incapacitante e terminal, com os seus cuidadores e familiares”.
In the curricular unit, Internship Report, on 1st Master’s Degree in Medical-Surgical Nursing, São José de Cluny Higher Nursing School, referring to the 2015-2017 academic year, emerge this report with its presentation and public discussion aims at the completion of the 2nd cycle of higher education leading to the master's degree. It describes all the accomplished path, ranging all clinical practice, from emergency and intensive care, to the prehospital optional practice. Focused on the common and specific nursing competences of a medical-surgical specialized nurse, within the its competences approaching critical care patient in pre and intra Hospital environment. The purpose is to point master level in medical-surgical nursing, through description, analysis and reflection of the performed and developed competences during all clinical practice. This aimed to parallel the scientific knowledge with experienced practical situations, considering that nursing knowledge is obtained from a constructive process, based on implicit and reflected practice. These practices were developed in the first internship - Emergency Room - and second - Polyvalent Intensive Care Unit - of SESARAM, E.P.E. The third and optional internship, was held at Instituto Nacional de Emergência Médica na Delegação Sul, Lisbon, in order to develop pre-hospital competences, considering critical care patient often triggers their illness process in pre-hospital environment. "Summarising, these clinicals practices, cemented elemental nursing skills and allowed the development of a new range of specialized competencies, such as "Care of patient experiencing complex illness processes in critical situation and/or organ failure", "Invigorate the response to catastrophe situations or multi-victim emergency, from design to action"; "Maximize the intervention in the prevention and infection control towards the person in critical situation and/or organic failure"; "Care of people with chronic, incapacitating and terminal illness, their caregivers and relatives" and "Establishes a therapeutic relationship with people with chronic disabling and terminal illness with their caregivers and relatives".
