Este relatório analisa a intervenção especializada de enfermagem à pessoa em situação
crítica submetida a pacemaker transvenoso provisório associado à Unidade Curricular Estágio final realizado numa Unidade de Cuidados Intensivos de nível 3.
A pessoa submetida a Pacemaker Transvenoso Provisório apresenta uma transição do tipo saúde/doença, pelo que o processo reflexivo deste relatório foi guiado pela Teoria da Transiçãode Meleis.
Neste contexto, foi desenvolvido um projeto de intervenção na área da intervenção
especializada de enfermagem para a pessoa em situação crítica submetida a Pacemaker
Transvenoso Provisório. Este tratamento é aplicado em casos de arritmias cardíacas com risco de vida e resistentes ao tratamento farmacológico, apresentando um elevado risco de complicações graves e de morte intra-hospitalar.
Com a intenção de promover a qualidade e segurança dos cuidados de enfermagem, o
projeto de intervenção foi orientado pela metodologia de projeto, promotora de uma prática baseada na evidência, e incluiu o diagnóstico de situação, a definição de objetivos, o planeamento, a execução e avaliação, com capacitação da equipa, a padronização das práticas e a promoção da continuidade dos cuidados, obtendo-se resultados positivos que são apresentados neste relatório.
O relatório aproveita para explorar e refletir sobre o desenvolvimento e aquisição de
competências do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica na área da Pessoa em Situação Crítica e de Mestre em Enfermagem.
This report analyzes the specialized nursing intervention for critically ill patients undergoing a temporary transvenous pacemaker, as part of the Curricular Unit Final Clinical Traineeship in a level 3 Intensive Care Unit. Patients undergoing a temporary transvenous pacemaker experience a health/illness transition, and the reflexive process of this report was guided by Meleis's Transition Theory. In this context, an intervention project was developed in the specialized nursing care área for critically ill patients undergoing a temporary transvenous pacemaker. This treatment is used in life-threatening cardiac arrhythmias that are resistant to pharmacological treatment and carries a high risk of severe complications and in-hospital mortality. To promote the quality and safety of nursing care, the intervention project was guided by the project methodology, which fosters evidence-based practice. It included situation assessment, objective setting, planning, execution, and evaluation, alongside team training, practice standardization, and continuity of care promotion, yielding positive results presented in this report. The report also explores and reflects on the development and acquisition of competencies of the Nurse Specialist in Medical-Surgical Nursing in Critical Care and the Master in Nursing.
