Abstract(s)
As Doenças Crónicas são doenças de longa duração e geralmente de progressão lenta, com
necessidade de cuidados e tratamentos continuados ao longo do tempo, o que representa,
atualmente, um dos principais desafios do século XXI. Este aumento crescente das doenças
crónicas e das incapacidades, limitações e alterações que provocam na vida das pessoas,
exigem cuidados de enfermagem especializados, que previnam a doença e que promovam a
adaptação e autogestão do regime terapêutico da pessoa em situação crónica. O presente
relatório, elaborado no âmbito do estágio de natureza profissional, módulo II, inserido no plano
de estudos do Mestrado em Enfermagem Médico-Cirúrgica na área da Pessoa em Situação
Crónica da Escola Superior de Enfermagem do Porto, emerge como um relato descritivo,
fundamentado e crítico-reflexivo do desenvolvimento das competências comuns e específicas
conducentes à especialização em Enfermagem. O estágio profissional teve como propósito o
desenvolvimento de competências através da concretização de um projeto individual de
desenvolvimento profissional, relevante para a prática clínica, baseada na evidência, do
enfermeiro especialista na área da Pessoa em Situação Crónica. Para tal, são incluídos neste
relatório dois casos clínicos experienciados, em que através do recurso à ontologia de
enfermagem, foi possível uma tomada de decisão especializada em situações concretas
relacionadas com a vivência da doença crónica, especificamente da Doença Renal Crónica.
Promover uma transição saudável e a promoção da autogestão do regime terapêutico da pessoa
foram os grandes propósitos dos cuidados especializados prestados. A concretização do projeto
individual de desenvolvimento profissional “Cuidar da Pessoa com Doença Renal Crónica”
permitiu, assim, desenvolver competências nesta área de especialidade, enquanto contribuiu
para a qualidade dos cuidados que procuram responder às verdadeiras necessidades da pessoa.
Chronic diseases are long-lasting and generally slow-progressing diseases that require ongoing care and treatment over time, which currently represents one of the main challenges of the 21st century. This growing increase in chronic diseases and the disabilities, limitations and changes they cause in people's lives require specialized nursing care that prevents disease and promotes adaptation and self-management of the therapeutic regimen in those with chronic conditions. This report, prepared within the scope of the professional internship, module II, included in the curriculum of the Master's Degree in Surgical Nursing in the area of the Person in Chronic Situation of Porto School of Nursing, emerges as a descriptive, grounded and critical-reflexive account of the development of the typical skills and competencies leading to specialization in Nursing. The goal of the professional internship was the development of specialist nurse skills through the implementation of an individual professional development project that was relevant to clinical practice and evidence-based, in the field of the Person in Chronic Situation. For this purpose, two clinical cases that were experienced are included in this report, in which through the use of nursing ontology, it was possible to make a specialized decision in real situations related to the experience of chronic disease, specifically Chronic Kidney Disease. Promoting a healthy transition and self-management of the person's therapeutic regimen were the main purposes of the specialized care provided. The implementation of the individual professional development project "Caring for the Person with Chronic Kidney Disease" thus allowed the development of skills in this area of specialisation, while contributing to the provision of quality care in response to the individual’s real needs.
