Abstract(s)
e a pessoa transplantada renal.
In Portugal, there is a high prevalence of people undergoing haemodialysis treatment due to chronic kidney disease. This situation involves complex treatment, as it involves demanding dietary restrictions and a complex medication regime. Knowing the challenges faced by this group of people, an internship report was developed with the aim of developing and promoting this theme. This report was drawn up as part of the Master's programme in Medical-Surgical Nursing, in the area of Nursing for People in Chronic Situations, at the Escola Superior de Enfermagem do Porto. Its purpose is to describe the process of developing and acquiring common and specific competences for the Specialist Nurse in the area of nursing for people in chronic situations, as recommended in regulations no. 140/2019 and no. 429/2018. During my master's degree, I was encouraged to develop a professional development project in a specific area of knowledge that was of interest to me. In this professional development project, a theme was chosen in the context of the chronically ill person. To this end, I outlined specific objectives and activities, each of which allowed me to carry out critical and reflective thinking and develop my skills in this area. This project is related to the person with renal impairment, more specifically, the medication regime for people on a regular haemodialysis programme and kidney transplant patients. People on a regular haemodialysis programme have a complex medication regime. As part of their care, nurses have privileged access to provide them with the knowledge and skills to be able to self-manage their clinical condition. The transplant patient must know and comply not only with the drug regime for immunosuppression, but also how to adopt the new lifestyle changes recommended after kidney transplantation.
