Abstract(s)
Cuidar da pessoa nos últimos dias/horas de vida (UDHV) e sua família é desafiante
para os enfermeiros, sendo por isso essencial implementar estratégias que os
capacitem a prestar cuidados de qualidade. A análise reflexiva acerca dos cuidados
prestados na Unidade de Cuidados Paliativos Agudos (UCPA), a análise dos dados
estatísticos epidemiológicos da mesma e dos registos de enfermagem e a aplicação
do questionário aos enfermeiros contribuíram para a identificação de lacunas na
promoção do conforto da pessoa nos UDHV e família. Assim, surgiu a questão: “Quais
são as intervenções de enfermagem promotoras de conforto da pessoa nos UDHV e
sua família numa Unidade de Cuidados Paliativos (UCP)?”. Este trabalho foi elaborado
segundo a metodologia de projeto com a finalidade de contribuir para a melhoria da
qualidade dos cuidados de enfermagem. De modo a basear a prática na evidência
científica, foi elaborada uma scoping review visando mapear as intervenções de
enfermagem promotoras do conforto, que foram complementadas com a observação
estruturada e participante durante a prestação de cuidados em estágio e a realização
de entrevistas semiestruturadas a enfermeiros peritos. Estas intervenções foram a
base para a elaboração de um Guia Orientador da Prática (GOP), que foi apresentado
à equipa de enfermagem através de ações de formação. O GOP encontra-se
estruturado com base nos contextos físico, psicoespiritual, sociocultural e ambiental
da Teoria do Conforto de Kolcaba que norteia este projeto, tendo as intervenções de
enfermagem sido organizadas com base nos mesmos. Deste modo, concluo que o
enfermeiro tem um papel importante na promoção do conforto da pessoa nos UDHV
e família, uma vez que a sua intervenção atende a todas as dimensões da pessoa.
Assim, este projeto contribuiu para a melhoria na qualidade dos cuidados na UCPA
através da elaboração de um instrumento de apoio à prática e da capacitação dos
enfermeiros.
Caring for the person in the last days/hours of life and their family is challenging for nurses, being so essential to implement strategies that enable them to provide quality care. The reflective analysis about the care provided in the Acute Palliative Care Unit, the analysis of its epidemiological statistical data and nursing records, and the application of the nurses' questionnaire contributed to the identification of gaps in the promotion of the comfort of the person in the last days/hours of life and family. So, the question: "What are the nursing interventions that promote the comfort of the person in last days/hours of life and their family in a Palliative Care Unit? This study was developed using the project methodology with the purpose of contributing to improving the quality of nursing care. In order to base the practice on scientific evidence, a scoping review was prepared to map comfort-promoting nursing interventions, which were complemented with structured and participant observation during the provision of internship care and semi-structured interviews with expert nurses. These interventions were the basis for the development of a Guiding Practice Guide, which was presented to the nursing team through training sessions. The Guiding Practice Guide is structured based on the physical, psychospiritual, sociocultural and environmental contexts of Kolcaba's Theory of Comfort, which guides this project, and nursing interventions were organized based on them. I conclude that nurses play an important role in promoting the comfort of the person in the last days/hours of life and family, since their intervention addresses all dimensions of the person. This project contributed to improving the quality of care at the Palliative Care Unit through the development of a practice support tool and the nurses' empowerment.
