Este Relatório visa refletir o percurso de aprendizagem no desenvolvimento de competências de Mestre e de Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica (EEESMO), ancorado na Teoria do Cuidar de Kristen Swanson.
Considerando que a maioria das mulheres sofrem lesões perineais durante o parto, o que pode impactar negativamente a saúde e bem-estar, assume especial relevância o estudo da relação entre as posições adotadas pela parturiente durante o período expulsivo, o resultado perineal e o seu impacto para a vivência de uma experiência de parto positiva.
A investigação foi conduzida em duas fases. Na primeira, uma Revisão Scoping, realizada conforme as diretrizes do Joanna Briggs Institute (2024), para mapear as evidências científicas sobre a influência da posição no período expulsivo e o resultado a nível perineal. Na segunda fase, foi desenvolvido um estudo descritivo e correlacional com abordagem quantitativa, envolvendo 31 EEESMOs e 30 parturientes. Foram aplicados dois questionários com os objetivos de: identificar as posições maternas adotadas no segundo estádio do trabalho de parto; descrever o resultado perineal e as práticas promotoras da sua integridade; conhecer a satisfação materna com a experiência de parto e analisar o contributo da posição materna no período expulsivo e do resultado perineal para uma experiência de parto positiva. A análise dos dados quantitativos foi realizada com o auxílio do Statistical Package for Social Science (SPSS), enquanto que, os dados qualitativos foram analisados com a técnica de análise de conteúdo proposta por Bardin (2016).
Os resultados revelaram, que a adoção de diversas posições no período expulsivo foi um contributo significativo para a satisfação das parturientes. Adicionalmente, identificou-se uma associação significativa entre a satisfação com o resultado perineal e a satisfação geral com a experiência de parto, evidenciando a importância de práticas que minimizem o trauma perineal. Evidenciou-se, uma correlação positiva e significativa entre a satisfação com o resultado perineal e a satisfação com as posições adotadas, assoalhando a importância da adoção de posições que promovam a integridade perineal. Por fim, não existe uma associação significativa entre as posições adotadas no período expulsivo e a ocorrência de lesão perineal, ressaltando a complexidade de outros fatores envolvidos no processo do parto.
This report aims to reflect the learning process in the development of Master's and Specialist Nurse in Maternal and Obstetric Health Nursing (EEESMO) competencies, anchored in Kristen Swanson's Caring Theory. Considering that most women suffer perineal injuries during childbirth, which can have a negative impact on their health and well-being, it is particularly important to study the relationship between the positions adopted by the parturient during the expulsive period, the perineal outcome and its impact on a positive childbirth experience. The research was conducted in two phases. In the first, a Scoping Review, carried out in accordance with the guidelines of the Joanna Briggs Institute (2024), to map the scientific evidence on the influence of position during the expulsive period and perineal outcome. In the second phase, a descriptive and correlational study was carried out with a quantitative approach, involving 31 EEESMOs and 30 parturients. Two questionnaires were used to: identify the maternal positions adopted during the second stage of labor; describe the perineal outcome and the practices that promote its integrity; find out about maternal satisfaction with the birth experience and analyze the contribution of the maternal position during the expulsive period and the perineal outcome to a positive birth experience. The quantitative data was analyzed using the Statistical Package for Social Science (SPSS), while the qualitative data was analyzed using the content analysis technique proposed by Bardin (2016). The results revealed that the adoption of different positions during the expulsion period was a significant contribution to the satisfaction of parturients. There was a positive and significant correlation between satisfaction with the perineal outcome and satisfaction with the positions adopted, highlighting the importance of adopting positions that promote perineal integrity. Finally, there was no significant association between the positions adopted during the expulsive period and the occurrence of perineal injury, highlighting the complexity of the other factors involved in the childbirth process.
