Abstract(s)
A prestação de cuidados a uma pessoa com necessidades de cuidados em saúde mental e psiquiátrica e/ou com Doença Mental Grave (DMG) é inexequível sem ter em conta todo o seu contexto individual, familiar e social.
Com o intuito de compreender o processo de adoecer mental no individuo e família e na DMG e de compreender a abordagem do Enfermeiro Especialista em Enfermagem de Saúde Mental e Psiquiátrica (EESMP) à pessoa e família, realizamos um estágio de natureza profissional que decorreu em três contextos clínicos distintos: num serviço de internamento de cuidados agudos, em contexto de cuidados diferenciados, num Hospital de Dia (HD), onde integramos a Equipa Local de Saúde Mental (ELSM) e em contexto comunitário numa Unidade de Cuidados na Comunidade (UCC). Após a integração nos serviços e respetivas equipas, participamos nas diferentes atividades desenvolvidas nos três locais de estágio, particularmente no apoio à família de forma individual e em grupo.
A participação em dois grupos distintos, grupo de Suporte Familiar, no grupo multifamiliar em contexto de HD, e um grupo Psicoeducativo de cuidadores familiares, em contexto comunitário na UCC; permitiu-nos perceber que para uma intervenção à família torna-se necessário que esta tenha um espaço onde possa livremente expor os seus problemas e dificuldades, e sentir-se acompanhada pelos seus pares e por um profissional de saúde, que para além, de um suporte emocional seja também, transmissor de conhecimentos sobre a doença, o tratamento, as estratégias comunicacionais, resolução de problemas e sobre recursos da comunidade disponíveis. Deste modo, os familiares conseguirão com mais facilidade criar estratégias de coping que lhes permitirá reformular o acompanhamento ao seu familiar com a DMG.
O trabalho desenvolvido com as famílias permitiu-nos constatar que a família deve ser integrada na intervenção terapêutica, como um parceiro de cuidados. Torna-se necessário que os serviços/equipas procurem dar suporte e apoio à família contribuindo para esta, adquira capacidades e competências para o processo de cuidar de um familiar com DMG, constituindo-se como um recurso para este, e para si própria enquanto unidade familiar. Este percurso possibilitou-nos também o desenvolvimento de competências como futuro EESMP.
Providing care to a person with mental health and psychiatric care needs and/or with Severe Mental Illness (GDM) is unfeasible without taking into account their entire individual, family and social context. In order to understand the process of mental illness in the individual and family and in GDM and to understand the EESMP approach to the person and family, we carried out a professional internship that took place in three distinct clinical contexts: in an inpatient care service and their teams, we participate in the different activities developed in the three internship sites, particularly in supporting the family individually and in groups. Participation in two distinct groups, the Family Support group, the multifamily group in the HD context, and a Psychoeducational group of family caregivers, in a community context in the CCU; It allowed us to understand that for an intervention to the family, it is necessary that they have a space where they can freely expose their problems and difficulties and feel accompanied by their peers and by a health professional who, in addition to emotional support, is also a transmitter of knowledge about the disease, treatment, communication strategies, problem solving, and available community resources. With this, family members will be able to more easily create coping strategies that will allow them to reformulate their experience with the various psychiatric pathologies and with GDM. The work developed with the families allowed us to verify that the family cannot be neglected from a therapeutic intervention, at the risk of mental illness spreading from the user to the whole family. It is necessary that the services/teams seek to provide support to the family, contributing to it, acquiring skills and competencies for the process of caring for a family member with GDM, constituting themselves as a resource for them, and for themselves as a family unit. This path also enabled us to develop skills as a future EESMP.
