No ciclo de vida, o ser humano está sujeito a situações de transição. Transições estas que quando não adaptativas podem provocar uma crise. A doença e a morte são situações de crise transacionais de perda necessitando de uma elaboração de luto. O luto corresponde a um processo natural, no entanto, quando não adaptativo apresenta implicações negativas na pessoa enlutada, em particular no luto prolongado.
A literatura refere como fatores para um luto prolongado a perda de um filho e a idade jovem do ente significativo.
O enfermeiro especialista em Saúde Infantil e Pediátrica tem a competência de cuidar da criança e família nos momentos de maior complexidade, nomeadamente na doença, na morte e no luto, respondendo às suas necessidades de adaptação.
Este relatório de estágio tem como objetivo apresentar e analisar de forma crítica, objetiva e contextualizada o percurso do estágio realizado em quatro diferentes contextos da prática (cuidados intensivos pediátricos, internamento de pediatria, cuidados na comunidade e cuidados intensivos neonatais) no âmbito do Curso de Mestrado em Enfermagem de Saúde Infantil e Pediatria e as competências comuns de Enfermeiro Especialista, específicas do Enfermeiro Especialista de Saúde Infantil e Pediátrica, bem como de Mestre adquiridas, com especial enfoque no tema “O Enfermeiro Especialista como gestor da crise causada pela doença crónica complexa e pela morte da criança”.
Neste percurso foram mobilizados conhecimentos de adaptação às situações complexas e facilitadores dos processos de luto, dando suporte à criança e família, com o intuito de responder às necessidades de cada elemento da família nas situações transacionais, promovendo assim um processo de adaptação às mudanças de saúde e em situação de morte, sustentado na Teoria de médio alcance de Afaf Meleis e no modelo de Cuidados Centrados na criança e família.
Throughout their life cycle, human beings are subject to situations of transition. When these transitions are not adaptive, they can cause a crisis. Illness and death are transitional crisis situations involving loss that require a grieving process. Grief is a natural process, but when it is not adaptive, it has negative implications for the bereaved person, particularly in cases of prolonged grief. The literature cites the loss of a child and the young age as factors contributing to prolonged mourning. Nurses specializing in Child and Pediatric Health have the competence to care for children and families in times of greatest complexity, namely in illness, death, and mourning, responding to their needs for adaptation. This internship report aims to present and analyze in a critical, objective, and contextualized manner the course of the internship carried out in four different practice contexts (pediatric intensive care, pediatric hospitalization, community care, and neonatal intensive care) within the scope of the Master's Degree in Child Health and Pediatric Nursing and the common skills of a Specialist Nurse, specific to the Specialist Nurse in Child Health and Pediatrics, as well as those acquired as a Master, with a special focus on the theme “The Specialist Nurse as manager of the crisis caused by complex chronic illness and the death of a child.” This process involved applying knowledge of how to adapt to complex situations and facilitate the grieving process, providing support to the child and family in order to respond to the needs of each family member in transitional situations, thereby promoting a process of adaptation to changes in health and in situations of death, based on Afaf Meleis's medium-range theory and Family-Centered Care.
