Abstract(s)
O presente relatório de estágio foi elaborado no âmbito da unidade curricular “Estágio de
Natureza Profissional com Relatório – Módulo II”, do plano de estudos do Curso de Mestrado em
Enfermagem de Reabilitação, da Escola Superior de Enfermagem do Porto.
Neste contexto, este relatório decorre de um projeto individual, elaborado no módulo I, e resulta
de um processo de formação avançada em enfermagem de reabilitação, que deriva de uma
aprendizagem de cariz reflexivo, nos diferentes contextos clínicos onde decorreram os estágios.
Essencialmente, este relatório visa demonstrar a transferência de conhecimentos e o
desenvolvimento de competências comuns do enfermeiro especialista e específicas do
enfermeiro especialista em enfermagem de reabilitação ao longo dos estágios, em diferentes
contextos clínicos. Não obstante, em consonância com o definido no projeto de estágio, optouse
pelo investimento na temática da fragilidade física, por forma a demonstrar o contributo do
enfermeiro especialista em enfermagem de reabilitação à pessoa idosa frágil, particularmente
no âmbito dos domínios da força muscular e do andar.
O investimento na fragilidade física visa acompanhar as alterações dos perfis epidemiológicos e
sociodemográficos da sociedade atual, que refletem o aumento da esperança média de vida, da
população idosa e da prevalência das doenças de evolução prolongada. Pesa embora a
longevidade em algumas pessoas não comprometa a sua condição de saúde, para outras motiva
o desenvolvimento de um estado de baixa reserva fisiológica, tornando-as vulneráveis a
resultados clínicos adversos e, portanto, frágeis.
Assim, neste relatório, numa primeira fase, são caracterizados os contextos clínicos onde
decorreram os estágios. Posteriormente, são apresentados dois estudos de caso que visam
demonstrar a conceção de cuidados do enfermeiro especialista em enfermagem de reabilitação
na pessoa idosa em condição de fragilidade física internada em contexto hospitalar, tendo por
base a ontologia de enfermagem. Em seguida, é feita a explanação do desenvolvimento das
competências comuns e específicas do enfermeiro especialista em enfermagem de reabilitação
e, no âmbito destas últimas, é abordado o contributo perante a pessoa idosa em condição de
fragilidade física internada em contexto hospitalar. Esse contributo ficou sistematizado num
programa de enfermagem de reabilitação. Por último, é efetuada a síntese final do relatório.
This report was conducted within the scope of the curricular unit “Professional Internship with Report – Module II”, of the study plan of the master’s course in Rehabilitation Nursing, at the Escola Superior de Enfermagem do Porto. In this context, this report results from an individual project, prepared in module I. It represents the outcomes of an advanced training experience in rehabilitation nursing, which derives from reflective learning gained across diverse clinical settings during the internships. The primary objective of this report is to showcase the knowledge transfer and the acquisition of general and specialized skills in rehabilitation nursing by specialist nurses through their participation in internships across various clinical settings. Thus, in line with what was defined in the internship project, the decision was made to focus on physical frailty. This choice aimed to illustrate the role of specialized rehabilitation nurses in supporting frail elderly individuals, specifically in improving muscular strength and walking abilities. The focus on physical frailty seeks to monitor changes in today's society's epidemiological and socio-demographic profiles. These changes are reflected in a rising life expectancy, a growing elderly population, and an increased prevalence of chronic diseases. While longevity may not affect health in some individuals, for others, it can lead to diminished physiological reserves, making them vulnerable to adverse clinical outcomes and thus susceptible to frailty. Therefore, this report is structured into several phases. Firstly, the clinical contexts of the internships are characterized. Following this, two case studies are presented to illustrate the care approach taken by specialized rehabilitation nurses towards elderly individuals experiencing physical frailty in a hospital setting, based on nursing ontology. Subsequently, the development of the common and specific competencies of the nurse specialist in rehabilitation nursing is explained and, within the scope of the latter, their impact on elderly patients with physical frailty in a hospital setting. This contribution was systematized in a rehabilitation nursing program. Finally, the report concludes with a summary of the key findings and insights.
