Abstract(s)
O doente crítico poderá necessitar de ventilação artificial, sendo o desmame
ventilatório a transição para o regresso a uma respiração espontânea. O Enfermeiro
Especialista em Enfermagem de Reabilitação (EEER), através da sua avaliação e
intervenção na reabilitação motora e respiratória, desempenha um papel importante na
melhoria da qualidade de vida, pelo que se considera um elemento preponderante neste
procedimento.
Esta dissertação surge no decurso do processo de aquisição de competências
específicas do EEER e obtenção do grau de Mestre em Enfermagem de Reabilitação
(ER), tendo sido realizada através das fases previamente programadas: definição do
tema; pesquisa inicial; elaboração de um protocolo e realização de uma scoping review
(ScR); e redação da dissertação. Esta dissertação teve como finalidade o
desenvolvimento de competências científicas e técnicas no processo do desmame
ventilatório difícil do doente crítico. Apesar da atuação do EEER assentar em
competências adquiridas e aprimoradas com a experiência, a existência de um protocolo
de desmame ventilatório, direcionado para o desmame difícil, assume relevância para a
uniformização da prática de ER, de acordo com evidência científica encontrada. O
objetivo geral desta dissertação consiste em recolher e analisar a melhor evidência
científica disponível sobre a intervenção de reabilitação no desmame ventilatório difícil
do doente crítico, construindo alicerces para o desenvolvimento de um protocolo de
atuação, com recurso à metodologia proposta pelo JBI® para ScR.
A revisão decorreu desde novembro de 2022 até março de 2023, tendo por base
3475 artigos mapeados e obtendo-se nove artigos, onde se identificaram intervenções
relevantes na reabilitação do doente crítico com desmame ventilatório difícil.
Considera-se pertinente mais estudos debruçados sobre esta problemática, de forma a
construir um protocolo de intervenção, realçando a atuação de ER
The critically ill patient may need artificial ventilation, with ventilatory weaning being the transition to spontaneous breathing. The Rehabilitation Nurse (RN) thought his assessment and intervention in motor and respiratory rehabilitation, plays an important role in improving the quality of life, and is therefore considered a preponderant element in this procedure. This dissertation arises during the process of acquiring specific competences of the RN and obtaining the Master´s degree in Rehabilitation Nursing, having been carried out through the previously programmed phases: definition of the theme; initial research; elaboration of a protocol and performance of a scoping review (ScR); and writing the dissertation. This dissertation aimed to develop scientific and technical skills in the process of difficult ventilatory weaning of critically ill patients. Despite the performance of RN based on skills acquired and improved with experience, the existence of a ventilatory weaning protocol, aimed at difficult weaning, assumes relevance for the standardizing the practice of Rehabilitation Nursing, according to scientific evidence found. The general objective of this dissertation is to collect and analyze the best scientific evidence available on rehabilitation intervention in difficult ventilatory weaning of critically ill patients, building foundations for the development of an action protocol, using the methodology proposed by JBI®, for ScR. The review took place from November 2022 to March 2023, based on 3575 mapped articles, resulting in nine articles, in which were identified relevant interventions were identified in the rehabilitation of critically ill patients with difficult ventilatory weaning. It is considered relevant to carry out further studies in this issue, in order to buid an intervention protocol, enhancing the role of rehabilitation nursing
