O presente relatório espelha o processo de desenvolvimento de aprendizagens pessoais e profissionais, ocorrido ao longo do 6º curso de mestrado e especialização na área de enfermagem de saúde infantil e
pediatria, cujo objetivo era desenvolver competências de enfermeiro especialista nessa área em contexto de comunidade, urgência e internamento pediátricos e unidade de cuidados intensivos neonatais. Este trabalho teve como base a evidência científica produzida e os documentos orientadores da profissão que, através da reflexão sobre a prática e das aprendizagens, me levaram a desenvolver um conjunto de competências que me permitiram compreender a dimensão do exercício profissional do enfermeiro especialista.
Utilizando a metodologia de projeto, parti de um problema, sentido a nível pessoal e institucional, que se prendeu com a necessidade de acompanhar as diretrizes que as entidades reguladoras em saúde, nacionais e internacionais, emanam a nível da realização do método canguru, mais concretamente a sua
abrangência a todos os recém-nascidos, no meu caso particular, a necessidade de uma verdadeira aposta na consolidação deste método na UCIN. Após esse passo, foi elaborado um plano de trabalho em que se definiram objetivos e atividades perspetivando resultados, quer a nível da consolidação da prática do
método canguru, quer a nível da promoção do desenvolvimento infantil saudável. As entidades reguladoras em saúde são claras em relação ao método canguru, defendendo que deve ser atingida uma taxa de cobertura universal, mesmo nos países em que esteja disponível uma avançada tecnologia hospitalar. Só assim serão atingidos resultados a nível da mortalidade infantil, da amamentação, da vinculação e do desenvolvimento infantil. Este método traz ainda, subsídios às diferentes transições experienciadas pela criança e família a nível hospitalar. Sendo um exemplo de cuidados para o desenvolvimento ao mesmo tempo que acompanha a filosofia dos cuidados centrados na família. Atribuo-lhe, ao longo deste trabalho, na perspetiva de Meleis, a designação de terapêutica de enfermagem facilitadora da experiência transicional que a criança e sua família vivem numa UCIN.
This report reflects the process of development of personal and professional learning, attained over the 6th master’s course in nursing in infant and pediatrics’ health specialization, whose goal was to develop specialist nursing skills in this area in the context of community, emergency and pediatric internment unit and neonatal intensive care unit. This work was based on the scientific evidence produced and the guiding documents of the profession, which, through reflection on practice and learning, led me to develop a set of skills that allowed me to understand the dimension of professional practice of the nurse specialist. Using the project methodology, I started from a problem, felt on a personal and institutional level, which was the need to follow the guidelines that emanate from the regulatory authorities in health, national and international, at the level of achievement of the kangaroo care, specifically its scope to all newborns, and in my particular case, the need for a real commitment to consolidation of this care in the NICU. After this step, a work plan was designed which defined objectives and activities foreseeing results, whether in terms of consolidation of the practice of the kangaroo care, and in terms of promoting healthy child development. Regulatory authorities in health are clear in regard to the kangaroo care, arguing that universal coverage rate should be achieved, even in countries where advanced hospital technology is available. Only then will the results be achieved at the level of infant mortality, breastfeeding, maternal-infant bonding and child development. This method brings further benefits to the different transitions experienced by the child and family in hospital. Being an example of developmental care while accompanying the philosophy of family centered care. I attribute it, throughout this work, and in the perspective of Meleis, the designation of nursing therapy enabling the transitional experiment that child and family go through while staying at a NICU
