O presente relatório, desenvolvido no âmbito do Mestrado em Enfermagem de Saúde Materna e Obstétrica, tem como finalidade descrever criticamente o percurso de desenvolvimento de competências especializadas do Enfermeiro Especialista, com particular enfoque na promoção, proteção e apoio ao aleitamento materno, bem como na possível integração da laserterapia de baixa potência enquanto intervenção autónoma e inovadora na prática clínica.
A amamentação constitui uma prática fundamental para o desenvolvimento saudável do recém-nascido, sendo recomendada em exclusivo até aos seis meses de vida. Contudo, apesar das elevadas taxas de iniciação, verifica-se uma redução significativa da sua continuidade, associada a múltiplos fatores de natureza biológica, psicológica e sociocultural, tais como dor, dificuldades na técnica de amamentação, apoio profissional insuficiente e condicionantes contextuais. Neste sentido, o papel do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica assume particular relevância na promoção de práticas baseadas na evidência e centradas nas necessidades da mulher e da família.
O relatório integra um enquadramento teórico sustentado na evidência científica atual e disponível, abordando a anatomofisiologia da lactação, os benefícios do leite materno, as principais dificuldades associadas ao aleitamento materno e as intervenções especializadas, com destaque para a laserterapia de baixa potência. Esta intervenção revela-se uma estratégia terapêutica segura e eficaz na redução da dor e na cicatrização de fissuras mamilares, contribuindo para a promoção do conforto materno, à luz da Teoria do Conforto de Kolcaba.
Ao longo dos diferentes contextos de estágio, cuidados de saúde primários, exames especiais de ginecologia e gravidez de alto risco, internamento de grávidas e puérperas, bem como urgência obstétrica e bloco de partos, foram desenvolvidas intervenções ajustadas às necessidades identificadas, nomeadamente no âmbito da educação para a saúde, capacitação das mulheres e famílias e formação das equipas de enfermagem.
Os resultados obtidos evidenciam ganhos em saúde, designadamente ao nível da melhoria do conforto materno, aumento da confiança na amamentação e promoção da sua
continuidade, reforçando a importância de uma prática de enfermagem especializada, baseada na evidência científica e centrada na pessoa.
This report, developed within the scope of the Master’s Degree in Maternal and Obstetric Health Nursing, aims to describe and critically reflect on the process of developing specialized competencies of the Specialist Nurse, with a particular focus on the promotion, protection, and support of breastfeeding, as well as on the potential integration of low-level laser therapy as an innovative intervention in clinical practice. Breastfeeding is a fundamental practice for the healthy development of the newborn and is recommended as exclusive feeding during the first six months of life. However, despite high initiation rates, there is a significant decline in its continuation, associated with multiple biological, psychological, and sociocultural factors, such as pain, difficulties in breastfeeding technique, insufficient professional support, and contextual constraints. In this context, the role of the Specialist Nurse in Maternal and Obstetric Health is particularly relevant in promoting evidence-based practices centered on the needs of the woman and her family. The report incorporates a theoretical framework grounded in current scientific evidence, addressing the anatomophysiology of lactation, the benefits of breast milk, the main challenges associated with breastfeeding, and specialized interventions, with particular emphasis on low-level laser therapy. This intervention has proven to be a safe and effective therapeutic strategy in reducing pain and promoting the healing of nipple fissures, thereby contributing to the enhancement of maternal comfort, in line with Kolcaba’s Theory of Comfort. Throughout the various clinical training settings, primary health care, specialized gynecological examinations, high-risk pregnancy care, inpatient care for pregnant and postpartum women, as well as obstetric emergency services and delivery rooms, interventions were developed and tailored to the identified needs. These included health education initiatives, the empowerment of women and their families, and the training of nursing teams. The results obtained demonstrate health gains, namely in terms of improved maternal comfort, increased confidence in breastfeeding, and the promotion of its continuation, reinforcing the importance of specialized nursing practice grounded in scientific evidence and centered on the person.
