Abstract(s)
O presente Relatório de Estágio surge no âmbito do 4º curso de Mestrado em
Enfermagem Médico-Cirúrgica da Escola Superior de Enfermagem S. José de Cluny e
pretende evidenciar todo o meu percurso de aquisição e desenvolvimento de Competências
do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica, através da descrição e
análise crítica das atividades desenvolvidas nas unidades curriculares de ensino clínico, que
decorreram cronologicamente no Serviço de Urgência (estágio I), na Unidade de Cuidados
Intensivos Polivalente (Estágio II), do Hospital Dr. Nélio Mendonça e na Unidade de
Cuidados Paliativos (Estágio III - opção), do Hospital Dr. João de Almada.
Embora distintos os contextos de estágio, em todos eles são exigidos cuidados
altamente complexos e especializados à pessoa em situação crítica e/ ou à pessoa em situação
paliativa e família, o que promoveu a aquisição de competências comuns e específicas do
enfermeiro especialista e de Mestre em Enfermagem Médico-Cirúrgica.
Os estágios I e II, tiveram como âmago, o desenvolvimento de competências
específicas no cuidado da pessoa a vivenciar processos complexos de doença crítica e/ou
falência orgânica; dinamizar a resposta a situações de catástrofe ou emergência multivítimas
e no maximizar a intervenção na prevenção e controlo da infeção perante a pessoa em
situação crítica e/ou falência orgânica.
O estágio III, constituiu a minha área de opção e permitiu o desenvolvimento das
competências na área de Enfermagem à pessoa em situação paliativa e aos seus
cuidadores/familiares com doença incurável ou grave, em fase avançada, progressiva e
terminal, especificamente no suporte e processo de adaptação às perdas sucessivas, à morte
e no acompanhamento no luto. Estas competências foram desenvolvidas em torno dos quatro
pilares fundamentais dos cuidados paliativos: controlo de sintomas, comunicação adequada,
apoio à família e trabalho em equipa, priorizando o alívio do sofrimento com o propósito de
preservar a dignidade, aumentar a qualidade de vida e maximizar o conforto da pessoa.
Concluído o percurso, considero que adquiri as competências acima mencionadas
de forma satisfatória, o que possibilitou uma prestação de cuidados especializados em
Enfermagem Médico-Cirúrgica, através de processos de tomada de decisão, com base na
evidência científica, na ação refletida, mantendo uma atualização contínua dos
conhecimentos científicos, tecnológicos e relacionais.
The present report comes related to the 4th Masters Degree in Medical-Surgical Nursing, from Escola Superior de Enfermagem S. José de Cluny, and intends to show my acquisition and development of skills as a Specialist Nurse in Medical Surgical Nursing, through the description and critical analysis of activities developed in curricular units of my clinical teaching practice, that occurred chronologically in Emergency Department(first practice), Critical Care Unit(second practice) of Hospital Dr. Nélio Mendonça and Palliative Care Unit (third practice) of Hospital Dr. João de Almada. Although practice sectors differ in context, it is demanded in all of them highly complex and specialized care, either to a critical patient and/or to a palliative patient and family, which promoted the acquisition of specific and mutual competences as a Specialist Nurse and Master in Medical Surgical Nursing. My first and second practice aim was the development of specific competences in caring for the patient going through complex stages of critical disease and/or organic failure; dynamizing response in calamity/emergency situations with multiple victims and also maximizing my intervention on preventing and controlling infection in a critical patient and/or organic failure. My third practice is my option area and allowed the development of competences in caring for the palliative patient and respective carers/families, with severe or incurable disease in advanced, progressive and terminal stages; specifically in support and adjusting process to continuous losses, to death and mourning support. These competences were developed around the four essential pillars of palliative care: symptoms control, proper communication, family support and team-work, putting suffering relieve first with the purpose of preserving dignity, increasing quality of life and maximizing patients comfort. Pathway concluded, I consider that I acquired the upper mentioned competences in a satisfactory way, which allowed me to provide specialized care in Medical Surgical Nursing, through decision making processes based on scientific evidence, on reflected actions and also on keeping a continuously updated scientific, technologic and relational knowledge.
