Este relatório insere-se na Unidade Curricular de Estágio com Relatório do 2º ano do 2º Curso de Mestrado em Enfermagem Médico-Cirúrgica, na área de especialização Enfermagem à Pessoa em Situação Crítica, da Escola Superior de Saúde da Cruz Vermelha Portuguesa de Lisboa. O documento descreve o percurso de aquisição de competências do enfermeiro especialista nesta área, bem como a consolidação das competências de mestre, em conformidade com os Regulamentos nº 140/2019, nº 429/2018 e nº 65/2018.
Os estágios decorreram em diferentes contextos: pré-hospitalar, em ambulância de Suporte Imediato de Vida, numa Unidade de Cuidados Intensivos polivalente e no Serviço de Urgência de um hospital centro de trauma. O objetivo geral foi desenvolver competências científicas, técnicas, éticas e relacionais na prestação de cuidados especializados à Pessoa em Situação Crítica, envolvendo também a família.
Foi dada especial atenção às práticas de enfermagem relacionadas com a gestão da dor em contexto crítico. Nesse sentido, foram desenvolvidos projetos para identificar o conhecimento e a perceção dos enfermeiros sobre esta problemática, promovendo o seu empoderamento através de formação e da implementação de projetos de melhoria contínua. O relatório salienta que o conhecimento e a perceção sobre a gestão da dor estão frequentemente baseados em crenças individuais e não em evidência científica, reforçando a necessidade de investimento institucional e liderança ativa neste domínio.
A estrutura do relatório organiza-se em dois capítulos: enquadramento teórico sobre a gestão da dor em contexto de situação crítica e a descrição do percurso de aquisição de competências de especialista e de mestre, sustentado por reflexão crítica e integração da prática baseada no conhecimento.
This report is part of the Internship with Report Curricular Unit of the 2nd year of the Master’s Degree in Medical-Surgical Nursing, specialization in Nursing Care for the Person in Critical Situation, at the Escola Superior de Saúde da Cruz Vermelha Portuguesa, Lisbon. The document describes the process of acquiring competencies as a specialist nurse in this field, as well as the consolidation of master’s-level competencies, in accordance with Regulations no. 140/2019, no. 429/2018, and no. 65/2018. The internships took place in different contexts: pre-hospital care in a Life Support ambulance, a polyvalent Intensive Care Unit, and the Emergency Department of a trauma center hospital. The general objective was to develop scientific, technical, ethical, and relational competencies in providing specialized care to the Person in Critical Situation, also involving the family. Special attention was given to nursing practices related to pain management in critical care settings. In this regard, projects were developed to identify nurses’ knowledge and perceptions about this issue, promoting their empowerment through training and the implementation of continuous improvement projects. The report highlights that knowledge and perceptions regarding pain management are often based on individual beliefs rather than scientific evidence, reinforcing the need for institutional investment and active leadership in this field. The structure of the report is organized into two chapters: the theoretical framework on pain management in the context of critical situations, and the description of the process of acquiring specialist and master’s-level competencies, supported by critical reflection and integration of knowledge-based practice.
