A melhoria dos outcomes da pessoa em situação crítica: a aplicação da bundle ABCDEF

| dc.description.abstract | O presente relatório final de estágio, surge no âmbito da Unidade Curricular de Estágio com Relatório do 2º Curso de Mestrado em Enfermagem Médico-Cirúrgica na área de especialização em Enfermagem à Pessoa em Situação Crítica da Escola Superior de Saúde da Cruz Vermelha Portuguesa – Lisboa. No relatório conta a análise crítica e reflexiva de todo o percurso realizado na aquisição e no desenvolvimento das competências comuns e específicas do enfermeiro especialista ao longo dos três estágios. Em cada local de estágio desenvolvi e apliquei um projeto de intervenção que teve como base a Metodologia de Projeto, fundamentado na prática baseada em evidência científica. O tema transversal explorado “A melhoria dos outcomes da pessoa em situação crítica: a aplicação da bundle ABCDEF”, baseia-se num conjunto estruturado de intervenções baseadas em evidência, composta por seis elementos: Assess, Prevent and Manage Pain; Both Spontaneous Awakening and Breathing Trials; Choice of Analgesia and Sedation; Delirium: Assess, Prevent and Manage; Early Mobility and Exercise; e Family Engagement and Empowerment. Uma vez que o Síndrome Pós-Internamento em Cuidados Intensivos persiste após a alta hospitalar e afeta entre 50 a 70% dos sobreviventes, a bundle permite uma orientação estruturada para a prestação de cuidados abrangentes e para a utilização otimizada dos recursos. Neste sentido, o enfermeiro como agente de mudança e prestador de cuidados reconhece a sua importância na melhoria dos outcomes, fazendo uso da bundle ABCDEF. O desenvolvimento do relatório teve por base a Teoria das Transições de Afaf Meleis, onde a transição vivenciada pela pessoa/família é o foco dos cuidados de enfermagem, sendo uma passagem despoletada por um evento crítico que leva a alterações tanto na pessoa como no seu ambiente. | por |
| dc.description.abstract | The present final internship report is produced within the scope of the Internship with Report course unit of the 2nd Master’s Program in Medical-Surgical Nursing, specializing in Critical Care Nursing at the Escola Superior de Saúde da Cruz Vermelha Portuguesa – Lisbon. The report presents a critical and reflective analysis of the entire process undertaken in the acquisition and development of the common and specific competencies of the specialist nurse throughout the three internships. At each internship site, I developed and implemented an intervention project based on Project Methodology and grounded in evidence-based practice. The cross-cutting theme explored, “Improving Outcomes for the Critically Ill Patient: The Application of the ABCDEF Bundle,” is based on a structured set of evidence-based interventions, composed of six elements: Assess, Prevent, and Manage Pain; Both Spontaneous Awakening and Breathing Trials; Choice of Analgesia and Sedation; Delirium: Assess, Prevent, and Manage; Early Mobility and Exercise; and Family Engagement and Empowerment. Given that Post-Intensive Care Syndrome persists after hospital discharge and affects between 50% and 70% of the survivors, the bundle provides a structured framework for delivering comprehensive care and optimizing resource use. In this context, the nurse, as both a change agent and care provider, recognizes their importance in improving patient outcomes through the application of the ABCDEF bundle. The development of the report was guided by Afaf Meleis’ Transition Theory, wherein the transition experienced by the patient and family is the focus of nursing care.
