Abstract(s)
A implementação do projecto “Pela Sua Saúde…
This paper intends to translate the development of a project that aims to contribute to the training of a group of users, with risk factor for cardiovascular disease, while promoting health and the adoption of healthy life styles. Both internship and project took place at a primary health care institution, between the 27th September 2010 and 11th February 2011. According to the General Health Department (2009) in Portugal, by 2006, circulatory system diseases were responsible for 32.23% of deaths. The approach to the users who suffered from a chronicle disease justifies a planned and organized act as well as the group effort of every professional on the identification of the changeable risk factors, in which life styles are included. Following the methodology of the Health Planning Process, on the first stage a diagnosis of situation was done, which allowed the characterization of a group of users and the identification of the health problem: physical inactivity. Prioritized health diagnosis and having selected the most appropriate strategies, to promote health and behavior change, it was proceeded to the elaboration and implementation of a community intervention project, which was named "For Your Health ... Move!" and whose main objective was to promote the adoption of regular habits of physical activity. The implementation of the project "For Your Health ... Move!" allowed the group to train users to practice regular physical activity. Integrated in the project were held with the group of users, a session on Health Education, a walk and three sessions of physical activity.
