Abstract(s)
No âmbito da unidade curricular de estágio de desenvolvimento profissional com relatório,
módulo II, incluída no Mestrado em Enfermagem Médico-Cirúrgica na área de Enfermagem à
Pessoa em Situação Paliativa, da Escola Superior de Enfermagem do Porto, desenvolvida no ano
letivo 2022/2023, foram realizadas 340h presenciais numa unidade de cuidados paliativos e
numa equipa intra-hospitalar de suporte em cuidados paliativos (170h em cada contexto). Este
estágio teve como objetivo o desenvolvimento de competências profissionais comuns e
específicas na área da enfermagem à pessoa em situação paliativa.
O presente relatório tem por objetivo descrever o percurso efetuado ao longo destas horas de
estágio. Foi elaborado segundo técnicas descritivas e de análise crítica, procurando descrever as
situações experienciadas, ao mesmo tempo que refletia sobre as mesmas, sempre em paralelo
com a análise da evidência científica disponível. O documento divide-se em três partes
principais, nomeadamente uma breve caracterização dos contextos clínicos onde se
desenvolveu o ensino clínico, a apresentação de dois casos da prática que foram trabalhados e
os contributos da prática para o desenvolvimento das competências profissionais.
Foram desenvolvidas e adquiridas competências comuns e específicas, nomeadamente, no
controlo de sintomas como a dor e a dispneia, na inclusão de familiares no processo de cuidar,
no acompanhamento nas perdas consequentes da doença e do luto, entre outras.
De um modo particular, foi trabalhado o tema da comunicação de más notícias. Estas definemse
como “qualquer informação que afeta seriamente e de forma adversa a visão de um
indivíduo sobre o seu futuro” (Baile et al., 2000, p.304). Segundo os mesmos autores, a forma
como as más notícias são transmitidas irá influenciar o modo como o doente/familiar
compreende a mensagem, a sua satisfação com o profissional de saúde, o seu nível de
esperança e a adaptação psicológica que se segue. Assim, com base tanto na pesquisa
bibliográfica efetuada, como na observação da prática, foram abordadas as necessidades e
reações dos doentes e seus familiares aquando da comunicação de más notícias, bem como as
dificuldades dos profissionais na comunicação deste tipo de informação. Foram, ainda,
abordadas algumas ferramentas desenvolvidas com o objetivo de auxiliar os profissionais de
saúde na transmissão de más notícias.
Todas estas competências adquiridas contribuíram para o meu desenvolvimento profissional e
pessoal.
Within the scope of the curricular unit of professional development internship with report, module II included in the Master's Degree in Medical-Surgical Palliative Care Nursing, from the Nursing School of Porto (Escola Superior de Enfermagem do Porto), developed in the academic year 2022/2023, an internship of 340 hours was carried out in a palliative care unit and in an inhospital palliative care support team (170 hours each context). This internship aimed to develop professional skills, common and specific, in nursing to the person in a palliative situation. The purpose of this report consists in describing the route taken during these hours of internship. It was done according to descriptive techniques and critical analysis, seeking to describe experienced situations, while reflecting on them, always in parallel with the analysis of available scientific evidence. The document is divided into three main parts, namely a brief characterization of the clinical contexts where the clinical teaching was developed, the presentation of two practical cases that were worked, and the contributions of practice to the development of professional skills. Common and specific skills were developed and acquired, namely in the control of symptoms such as pain and shortness of breath, in including family members in the care process, in monitoring losses resulting from illness and mourning, among others. In particular, the issue of communicating bad news was worked on. These are defined as “any information which adversely and seriously affects an individual’s view of his or her future” (Baile et al., 2000, p.304). According to the same authors, the way bad news are conveyed will influence how the patient/family member understands the message, their satisfaction with the healthcare professional, their level of hope and the psychological adaptation that follows. Thus, based on both the bibliographic research carried out, as in the observation of practice, the needs and reactions of patients and their families when bad news were communicated, as well as the difficulties of professionals in communicating this type of information were assessed. There were still some tools addressed, developed with the aim of helping healthcare professionals in delivering bad news. All these acquired skills contributed to my professional and personal development
