A promoção ao regime terapêutico é um tema que tem vindo a ser estudado
e trabalhado pelos enfermeiros ao longo dos anos. Os utentes com patologia
psiquiátrica apresentam níveis de não adesão ao regime terapêutico muito
elevados, em comparação com os utentes com patologia orgânica. Fatores como a
presença de sintomas psicóticos, ausência de insight, défices cognitivos contribuem
para a não adesão terapêutica que consequentemente resulta no agravamento de
sintomas, reinternamentos, recaídas, maiores défices cognitivos e diminuição da
qualidade de vida. A intervenção do enfermeiro especialista na promoção da adesão
terapêutica irá ser fundamental, de forma a minimizar as consequências de
ausência de tratamento, melhorar o prognóstico, minimizar os riscos de uma nova
recaída, melhorar a qualidade de vida e diminuir as hospitalizações. Delinear um
projeto de intervenção que promova a adesão ao regime terapêutico torna-se
fundamental e prioritário de forma a melhorar prognósticos e a prevenir sequelas
incapacitantes. Foram definidos como objetivos deste projeto de intervenção:
Identificar comportamentos de não adesão ao regime terapêutico e compreender
os motivos de não adesão ao regime terapêutico e promover a adesão ao regime
terapêutico com recurso a intervenções psicoterapêuticas e psicoeducativas,
utilizando a entrevista motivacional e os mediadores expressivos como estratégia,
recurso facilitador e promotor de mudança de comportamento. O projeto de
intervenção foi aplicado na população adulta de ambos os sexos, entre os 18 e os
82 anos de idade. Os resultados das intervenções são qualitativos com base em
feedback dos utentes, dos profissionais de saúde, da observação comportamental
e dos registos elaborados. Os resultados obtidos comprovam a eficácia das
intervenções psicoterapêuticas e psicoeducacionais, tendo havido melhores
resultados no contexto de internamento em comparação com contexto de Hospital
de Dia, onde os utentes mostraram maior ambivalência.
Promoting the therapeutic regimen is a topic that has been trained and worked on by nurses over the years. Users with psychiatric pathology have very high levels of non-adherence to the therapeutic regimen, compared to users with organic pathology. Factors such as the presence of psychotic symptoms, lack of insight, cognitive deficits reduced to therapeutic non-adherence which consequently result in worsening of symptoms, readmissions, relapses, greater cognitive deficits, and decreased quality of life. The intervention of the specialist nurse in promoting therapeutic adherence will be fundamental, to minimize the consequences of the absence of treatment, improve the prognosis, minimize the risks of a new recovery, improve the quality of life, and reduce hospitalizations. Outlining an intervention project that promotes adherence to the therapeutic regimen becomes fundamental and a priority to improve predictions and prevent disabling sequelae. The objectives of this intervention project were defined as: identifying behaviors of non-adherence to the therapeutic regimen and understanding the reasons for non-adherence to the therapeutic regimen and promoting adherence to the therapeutic regimen using psychotherapeutic and psychoeducational interventions, using motivational interviewing and expressive mediators as a strategy, facilitating resource and promoter of behavior change. The intervention project was applied to the adult population of both sexes, between 18 and 82 years of age. Intervention results are qualitative based on feedback from users, healthcare professionals, behavioral observation, and reviewed records. The results obtained proved the effectiveness of psychotherapeutic and psychoeducational interventions, having obtained better results in the inpatient context compared to the Day Hospital context, where users demonstrated greater ambivalence.
