Este documento reflete todo o trabalho clínico desenvolvido no âmbito do estágio de natureza profissional integrado no curso de Mestrado em Enfermagem Médico-Cirúrgica na área de Enfermagem à pessoa em situação paliativa. O estágio teve por objetivo o desenvolvimento de competências clínicas comuns e especializadas, na área da Enfermagem paliativa. O luto preparatório em clientes em fim de vida foi a área de intervenção privilegiada com o intuito de obter, aprofundar conhecimento de como aliviar o sofrimento humano inerente às perdas de fim de vida, independentemente da ordem destas, físicas, psicológicos, emocionais, sociais ou espirituais. Metodologia: Utilizamos uma metodologia descritiva-reflexiva de relato dos factos/atividades, suportada pela melhor evidência disponível, e recurso aos regulamentos da Ordem dos Enfermeiros sobre as competências para a área especializada da Enfermagem. Realizamos uma revisão da literatura com base nos princípios metodológicos do Joanna Briggs Institute com os objetivos: mapear as intervenções desenhadas para o luto preparatório em clientes em fim de vida e examinar os resultados sobre a utilização da intervenção dignity talk nestes clientes. Resultados: As intervenções para a luto preparatório têm por base a comunicação, a disponibilidade e a escuta ativa atendendo às narrativas do cliente. A intervenção dignity talk promove conversas importantes de fim de vida, o sentido de valor e a dignidade da pessoa, promove a interação eficaz e a resolução de assuntos pendentes, facilitando o processo de luto. O enfermeiro de cuidados paliativos é o profissional de saúde que está numa posição privilegiada na equipa multidisciplinar, para avaliar e cuidar da pessoa doente e da família, daí a importância de obter um perfil de competências especializadas, que melhorem a qualidade da assistência em fim de vida de forma contínua.
Introduction: This document reports and reflects all the clinical work developed within the scope of the professional internship integrated into the Master's degree in Medical-Surgical Nursing in the area of Nursing for people in palliative situations. The internship aimed to develop common and specialized clinical skills in the area of palliative nursing. Preparatory grief in end-of-life clients was the privileged area of intervention with the aim of obtaining knowledge of how to alleviate the human suffering inherent to significant losses, regardless of their order, physical, emotional, cognitive, social or spiritual. Methodology: We use a reflective descriptive methodology to report facts/activities, supported by the best available evidence, using what is available in the Nurses' Association regulations on competencies. We carried out a literature review based on the methodological principles of the Joanna Briggs Institute with the objectives: to map the interventions designed for preparatory grief in clients at the end of life and examine the literature results about the use of the dignity talk intervention with these clients. Results: Interventions for preparatory grief is based on communication, availability and active listening. The dignity talk intervention promotes important end-of-life conversations, the person's sense of value and dignity, promotes effective interaction and the resolution of outstanding issues, facilitating the grieving process. The palliative care nurse is the health professional who is in a most prominent position in the multidisciplinary team, to assess and care for the patient and their family, hence the importance of obtaining specific skills, a profile of specialized skills for improvement continuous quality of end-of-life care.
