- Avaliação do dano psíquico no âmbito do Direito CivilPublication . Melo, Bruna de; Bettencourt, Mariana Mendonça; Colón, Máximo Fernandez; Henriques, Carla; Matos, AnaA avaliação do dano em sede de Direito Civil tem por objetivo a reparação integral de todos os danos emergentes da ocorrência, tanto patrimoniais como não patrimoniais. No âmbito específico da Psiquiatria Forense, a avaliação de um dano pós-traumático reveste-se de especial dificuldade pela singularidade e subjetividade dos aspetos clínicos ligados a esta especialidade. Pretende-se, com este trabalho, uma descrição clínica e sócio-demográfica dos indivíduos avaliados no âmbito da reparação do dano em Direito Civil, no contexto Psiquiátrico-forense, na Delegação do Centro do INMLCF e no Gabinete Médico-Legal Dão-Lafões, no período decorrido entre 2015 e 2018. Pretende-se também avaliar a relação entre os diagnósticos estabelecidos e a valorização. Para este efeito, foi realizada uma revisão não sistemática da literatura sobre o tema, bem como consulta dos processos médico-legais das 102 pessoas avaliadas. Foi realizada uma descrição da amostra e posteriormente avaliada a associação entre a valorização do dano e várias variáveis sociodemográficas e clínicas. Em 12.7% das avaliações foi feito o diagnóstico de Perturbação de Stress Pós-Traumático, em 10.8% de Perturbações de adaptação e em 6.9% como Reações ao Stress severo. Em 24.5% dos casos não foi diagnosticada qualquer doença, pelo que não houve valorização. O grupo de pessoas diagnosticadas com " Perturbações de stress pós-traumático" teve tendencialmente mais elevadas do que os restantes grupos A maioria dos eventos desencadeantes foram acidentes de viação, em concordância com a literatura. A maior valorização da Perturbação de Stress Pós-Traumático pode ser compreendida pela incapacidade que acarreta. Em futuros estudos, seria importante fazer uma análise mais extensa do estado pré-mórbido do examinado, como a personalidade prévia ou psicopatologia anterior.
- Pre-hospital treatment of acute trauma pain: an observational studyPublication . Mota, Mauro; Reis Santos, Margarida; Santos, Eduardo; Henriques, Carla; Matos, Ana; Cunha, MadalenaObjective: To describe and analyze the effectiveness of nurses’ interventions in pain reduction among patients with traumatic injury. Methods: Prospective cohort study conducted in the Immediate Life Support Ambulances in Portugal from March 1, 2019 to April 30, 2020. We have collected data on the kind of interventions implemented and the time elapsed during rescue procedures. To investigate the course of acute trauma pain, a 11-point Numeric Rating Scale was used. Changes in the level of pain registered throughout the three assessment moments were studied using linear mixed-effects models with random intercepts to account for the repeated measurements conducted on the same patient. These changes were assessed before and after the administration of the pain relief interventions. Results: 596 patients were included in this study. Most of them were male (65.9%) and had a mean age of 53.05±19.72 years. There was a reduction in the average pain intensity of 2.44 points (p<0.005), between the beginning and end of the assessment, and a reduction of 39.62% among the patients who were experiencing a level of pain equal to or greater than 7 (46.7% vs 7.08%, p<0.05). Measures involving the use of morphine, cryotherapy and relationship-based measures have proven to be effective. Comfort measures as a whole do not seem to have a significant impact on pain relief. Conclusion: Pre-hospital pharmacological and non-pharmacological nurses’ interventions have proven to be effective in reducing pain. Comfort measures have not been proved to be effective, so their potential must be rethought and enhanced.
- Psychological injury assessment under civil lawPublication . Melo, Bruna; Bettencourt, Mariana Mendonça; Colón, Máximo Fernandez; Henriques, Carla; Matos, AnaThe objective of injury assessment under Civil Law is to fully compensate all injuries arising from the occurrence, both property and non-property (Article 562 of the Civil Code - " whoever is obliged to compensate an injury must reconstitute the situation that would have existed if the event requiring compensation had not occurred ). In the specific field of Forensic Psychiatry, post-traumatic injury assessment is particularly difficult due to the singularity and subjectivity of the clinical aspects linked to this specialty. The aim of this work is to provide a clinical and socio-demographic description of the individuals assessed in the scope of compensation of injury under Civil Law, in the psychological-forensic context, at the INMLCF Center Delegation, as well as in the Medical-Legal Office Dão-Lafões, in the time period between 2015 and the end of 2018. It is also intended to assess the relationship between the established diagnoses and the valuation. Bearing this purpose in mind, a non-systematic review of the literature on the topic was carried out, as well as consultation of the medico-legal processes concerning the 102 individuals assessed. A description of the sample was carried out, and the association between injury valuation and various sociodemographic and clinical variables was subsequently assessed. In 12.7% of the assessments the diagnosis of Post-Traumatic Stress Disorder was made, in 10.8% of adjustment Disorders and in 6.9% of Other Reactions to Severe Stress Disorders The group of individuals diagnosed with "post-traumatic stress disorder" had valuations which tended to be higher than the other groups. The majority of the traumatic events were road accidents, in line with the numbers found in the literature. The higher valuation of Post-traumatic Stress Disorder might be explained by the disability its symptoms entail. In future studies, it would be important to make a more extensive analysis of examined individual's premorbid state, such as their previous personality or previous psychopathology.
