A dor pélvica aguda na mulher é uma causa frequente de admissão na urgência hospitalar.
Os exames radiológicos permitem diagnosticar as patologias ginecológicas agudas, as quais são caracterizadas por dor súbita nos
quadrantes abdominais inferiores, febre, meno ou metrorragias, hemorragia intra-peritoneal ou sintomas de choque.
A ecografia é a primeira técnica radiológica na avaliação da dor pélvica aguda na mulher, embora por vezes não seja conclusiva.
A TC e a RM são efectuadas se a ecografia for equívoca, se a alteração se extender para além do campo de visão observável pela
sonda transvaginal ou se for necessária uma melhor caraterização da doença. A RM é complementar à ecografia nos casos em que
o contraste iodado e a radiação não são desejáveis.
São várias as alterações ginecológicas que causam dor pélvica aguda, incluindo alterações do útero, ovário, endometriose, doença
inflamatória pélvica, alterações relacionadas com a gravidez e complicações pós-parto ou pós-operatórias.
Um diagnóstico rápido e preciso é essencial para o tratamento adequado das alterações pélvicas agudas. É importante diferenciar
as doenças que requerem uma abordagem cirúrgica ou intervencional imediata, de condições que podem ser tratadas de forma
conservadora.
Acute pelvic pain in women is a frequent cause of emergency hospital admission. Imaging techniques have enabled diagnosis of acute gynecologic conditions, which are characterized by sudden onset of lower abdominal pain, fever, genital bleeding, intraperitoneal bleeding or symptoms of shock. US is the first imaging tool in the assessment of acute pelvic pain in female patient, however usually is not conclusive. TC and MR are performed if US findings are equivocal, if the abnormality extends beyond the field of view achievable with the transvaginal probe or if further characterization of pelvic disease is required. MR is also a complement to US when iodinated contrast media or radiation is not desirable. Many gynecologic conditions cause acute pelvic pain, including uterine disorders, ovarian disorders, endometriosis, pelvic inflammatory disease, pregnancy related conditions, postoperative or postpartum complications. Rapid and accurate diagnosis is essential for the appropriate management of acute pelvic conditions. It is important to differentiate diseases that require an immediate surgical or interventional approach, from conditions that can be treated with conservative therapy.
