A promoção de cuidados de saúde centrados na mulher e na família no período pós-parto tem vindo a assumir crescente relevância, dada a complexidade das transições vivenciadas nesta fase do ciclo gravídico-puerperal. Este período constitui um momento crítico, marcado por alterações físicas, emocionais, sociais e familiares, que exigem adaptação e reorganização, sendo essencial o desenvolvimento de estratégias que promovam o empoderamento e a autonomia da mulher e família.
O Plano de Pós-Parto é a temática que sustenta o presente relatório enquanto intervenção promotora do empoderamento da mulher e família, tendo como referencial teórico a Teoria das Transições de Afaf Meleis, que permite compreender os processos adaptativos inerentes às transições vividas no pós-parto e orientar a intervenção do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica (EEESMO).
Foram definidos como objetivos: refletir sobre o desenvolvimento de competências comuns, específicas do EEESMO e de Mestre ao longo dos ensinos clínicos; e evidenciar as atividades implementadas nos diferentes contextos de estágio com vista à promoção e implementação do PPP.
Ao longo dos diversos contextos de estágio — bloco de partos, internamento de puerpério, medicina materno-fetal, exames especializados e cuidados de saúde primários — foram desenvolvidas atividades centradas na capacitação da mulher e família, na promoção da continuidade de cuidados e na sensibilização para a importância do PPP. Destacam-se a realização de diagnósticos de situação, o desenvolvimento de materiais educativos, a implementação de sessões de formação dirigidas a profissionais de saúde e a mulheres e famílias, bem como a elaboração de instrumentos estruturados como o “Plano de Pós-Parto” e o “Guia Orientador do Pós-Parto”. Este percurso contribuiu para o desenvolvimento de competências científicas, técnicas, éticas e relacionais, sustentadas na evidência científica, evidenciando o PPP como uma ferramenta facilitadora da transição para a parentalidade, promotora do envolvimento ativo, da tomada de decisão informada e da continuidade dos cuidados.
Assim, a obtenção do grau de Mestre e do título de EEESMO permite assegurar cuidados especializados, centrados na mulher e família, contribuindo para a melhoria da qualidade dos cuidados e para a promoção de transições saudáveis.
The promotion of woman and family-centered healthcare in the postpartum period has gained increasing relevance, given the complexity of the transitions experienced during this phase of the pregnancy–puerperal cycle. This period constitutes a critical moment, marked by physical, emotional, social, and family changes that require adaptation and reorganization, making it essential to develop strategies that promote the empowerment and autonomy of women and their families. The Postpartum Plan underpins this report as an intervention aimed at promoting the empowerment of women and families, based on the Transitions Theory of Afaf Meleis, which enables the understanding of adaptive processes inherent to postpartum transitions and guides the intervention of the midwife. The objectives defined were: to reflect on the development of common, specific midwifery and Master’s-level competencies throughout clinical placements; and to highlight the activities implemented in different clinical contexts aimed at promoting and implementing the Postpartum Plan. Throughout the various clinical settings —delivery room, postpartum ward, maternal-fetal medicine, specialized examinations, and primary healthcare — activities were developed focusing on empowering women and families, promoting continuity of care, and raising awareness of the importance of the Postpartum Plan. These included situation diagnoses, the development of educational materials, the implementation of training sessions directed at healthcare professionals as well as women and families, and the creation of structured tools such as the “Postpartum Plan” and the “Postpartum Handbook”. This process contributed to the development of scientific, technical, ethical, and relational competencies, grounded in scientific evidence, highlighting the Postpartum Plan as a facilitating tool for the transition to parenthood, promoting active involvement, informed decision-making, and continuity of care. Thus, obtaining the Master’s degree and the title of midwife enables the provision of specialized, woman and family-centered care, contributing to improved quality of care and the promotion of healthy transitions.
