| dc.description.abstract | Introdução: A imagem médica desempenha um papel fundamental no diagnóstico e tratamento de diversas patologias, sendo essencial garantir a qualidade das imagens e a segurança dos doentes. O uso de radiação ionizante exige uma abordagem baseada nos princípios da proteção radiológica (justificação, otimização e limitação de dose). Os níveis de referência de diagnóstico (NRD) surgem como ferramentas essenciais para a monitorização e para o controlo da dose, permitindo identificar discrepâncias e promover a otimização dos protocolos institucionais. Objetivo: Avaliar e analisar os valores de exposição radiológica praticados no Hospital Internacional de Omã, nas modalidades de radiologia geral, mamografia e tomografia computorizada (TC), e comparar os mesmos com os níveis de referência de diagnóstico estabelecidos a nível internacional e local. Métodos: Realizou-se um estudo observacional, retrospetivo, quantitativo e descritivo, com base em dados de exames realizados entre junho de 2024 e junho de 2025. A amostra incluiu 310 doentes adultos, correspondendo a 660 procedimentos nas três modalidades em estudo. Os parâmetros de aquisição e os indicadores de dose analisados variaram de acordo com a modalidade de imagem. Os dados foram recolhidos a partir do Picture Archiving and Communication System (PACS) e da plataforma Teamplay™ (Siemens Healthineers), sendo posteriormente analisados no software International Business Machines Statistical Package for the Social Sciences (IBM SPSS) Statistics®, com o cálculo de medidas estatísticas descritivas. Foram analisados os principais descritores de dose por modalidade, nomeadamente o Kerma-Area Product (KAP) na radiologia geral, a Average Glandular Dose (AGD) na mamografia e, na tomografia computorizada, o Computed Tomography Dose Index (CTDI), o Dose Length Product (DLP) e a Size Specific Dose Estimate (SSDE), sendo, posteriormente, comparados com os NRD disponíveis. Resultados: Na radiologia geral, os valores médios de KAP variaram por região anatómica, tórax (0,14 Gy.cm²), coluna cervical (0,15 Gy.cm² em ântero-posterior (AP) e 0,22 Gy.cm² em lateral (LAT)), coluna lombar (3,04 Gy.cm² em AP e 3,83 Gy.cm² em LAT), joelhos em carga (0,17 Gy.cm²), seios perinasais (0,54 Gy.cm²), pés (0,03 Gy.cm² em AP e 0,04 Gy.cm² em oblíquo (OBL)) e mãos (0,03 Gy.cm² em PA e OBL). Na mamografia, a AGD por tipo de aquisição foi de 1,26 mGy para a crânio-caudal direita, 1,24 mGy para a crânio-caudal esquerda, 1,39 mGy para a oblíqua medio-lateral direita e 1,37 mGy para a oblíqua medio-lateral esquerda, correspondendo a AGD total de 5,26 ± 1,04 mGy. Na tomografia computorizada, nas fases de aquisição, os valores médios de CTDI foram de 36,32 mGy para o crânio, 19,95 mGy para os seios perinasais, 6,05 mGy para o tórax e 6,32 mGy para as vias urinárias. O DLP total apresentou valores médios de 682,02 mGy.cm para o crânio, 308,46 mGy.cm para os seios perinasais, 214,32 mGy.cm para o tórax e 291,45 mGy.cm para as vias urinárias. A SSDE apresentou valores médios de 40,28 mGy para o crânio, 18,99 mGy para os seios perinasais, 8,81 mGy para o tórax e 10,18 mGy para as vias urinárias. Conclusão: Os valores de exposição radiológica praticados no Hospital Internacional de Omã encontram-se, de forma geral, em conformidade com os NRD, evidenciando uma prática segura e alinhada com os princípios da proteção radiológica. Estes resultados irão potenciar a otimização dos procedimentos reduzido a exposição dos doentes, e consequentemente os riscos inerentes. | por |
| dc.description.abstract | Introduction: Medical imaging plays a fundamental role in the diagnosis and treatment of various diseases, and it is essential to ensure image quality and patient safety. The use of ionizing radiation requires an approach based on the principles of radiation protection (justification, optimization and dose limitation). Diagnostic Reference Levels (DRL) serve as essential tools for monitoring and controlling dose, enabling the identification of discrepancies and promoting the optimization of institutional protocols. Objective: To evaluate and analyze the radiation exposure values practiced at the International Hospital of Oman in the modalities of general radiology, mammography, and computed tomography (CT), and to compare them with the diagnostic reference levels established at the international and local levels. Methods: We conducted a retrospective, quantitative, and descriptive observational study based on data from examinations performed between June 2024 and June 2025. The sample included 310 adult patients, corresponding to 660 procedures across the three imaging modalities under study. The acquisition parameters and dose indicators analyzed varied according to the imaging modality. Data were collected from the Picture Archiving and Communication System (PACS) and the Teamplay™ platform (Siemens Healthineers) and subsequently analyzed using the International Business Machines Statistical Package for the Social Sciences (IBM SPSS) Statistics® software, with the calculation of descriptive statistical measures. The main dose descriptors were analyzed by modality, namely the Kerma-Area Product (KAP) in general radiology, the Average Glandular Dose (AGD) in mammography, and, in computed tomography, the Computed Tomography Dose Index (CTDI), the Dose Length Product (DLP) and the Size Specific Dose Estimate (SSDE), which were subsequently compared with the available NRDs. Results: In general radiology, the mean KAP values varied by anatomical region: chest (0.14 Gy·cm²), cervical spine (0.15 Gy·cm² in anteroposterior (AP) views and 0.22 Gy·cm² in lateral (LAT) views), lumbar spine (3.04 Gy·cm² in AP views and 3. 83 Gy·cm² in LAT), weight-bearing knees (0.17 Gy·cm²), paranasal sinuses (0.54 Gy·cm²), feet (0.03 Gy·cm² in AP and 0.04 Gy·cm² in oblique (OBL)), and hands (0.03 Gy·cm² in PA and OBL). In mammography, the Dose per Unit Area (DPA) by acquisition type was 1.26 mGy for the right craniocaudal, 1.24 mGy for the left craniocaudal, 1.39 mGy for the right mediolateral oblique, and 1.37 mGy for the left mediolateral oblique, corresponding to a total AGD of 5.26 ± 1.04 mGy. In computed tomography, during the acquisition phases, the mean CTDI values were 36.32 mGy for the skull, 19.95 mGy for the paranasal sinuses, 6.05 mGy for the chest, and 6.32 mGy for the urinary tract. The total DLP showed mean values of 682.02 mGy·cm for the skull, 308.46 mGy·cm for the paranasal sinuses, 214.32 mGy·cm for the chest, and 291.45 mGy·cm for the urinary tract. The SSDE showed mean values of 40.28 mGy for the skull, 18.99 mGy for the paranasal sinuses, 8.81 mGy for the chest, and 10.18 mGy for the urinary tract. Conclusion: The radiation exposure levels at the International Hospital of Oman are generally in compliance with the DRL, indicating safe practices that align with the principles of radiation protection.
