A malnutrição associada à recusa alimentar é frequente nas pessoas idosas
com demência e é de etiologia multifatorial. A alimentação entérica continua a ser uma
das vias de alimentação preferenciais nos cuidados às pessoas idosas com demência,
apesar das orientações internacionais da European Society for Clinical Nutrition and
Metabolism e do National Institute for Health and Care Excellence indicarem não
existir suporte científico que comprove a sua eficácia. Esta situação confronta a equipa
de enfermagem com decisões sobre questões éticas importantes sobre como
alimentar estas pessoas e como esta perceção influencia a tomada de decisão no seio
da equipa multidisciplinar.
Os objetivos delineados para o Estágio com Relatório foram: prestar cuidados
à pessoa idosa com demência em situação de cuidados complexa; promover
formação em serviço à equipa de enfermagem e outros profissionais de saúde sobre
alimentação nas pessoas idosas com demência e descrever as experiências e
perspetivas dos enfermeiros de uma unidade de demências sobre a alimentação nas
pessoas idosas com demência através de um estudo qualitativo.
No estudo participaram 7 enfermeiros de uma unidade de demências em
Lisboa, tendo sido realizadas entrevistas semiestruturadas e os discursos registados
foram analisados utilizando a técnica de análise de conteúdo.
Como conclusões principais, destacam-se: a alimentação das pessoas idosas
com demência depende de uma avaliação individualizada e de um plano
personalizado; o enfermeiro é o principal interveniente nas estratégias de alimentação
e o elo entre a equipa multidisciplinar e os cuidadores familiares; a necessidade de
disseminar as orientações internacionais sobre alimentação nas pessoas idosas com
demência no seio da equipa de enfermagem e de incrementar a sua discussão.
Malnutrition associated with food refusal is frequent in elderly people with dementia and has a multifactorial etiology. Enteral nutrition continues to be one of the preferred feeding paths in the care of elderly people with dementia, despite the international guidelines from European Society for Clinical Nutrition and Metabolism and National Institute for Health and Care Excellence indicating that there is no scientific support to prove its effectiveness. This situation confronts the nursing team with decisions on important ethical issues about how to feed these people and how this perception influences decision-making within the multidisciplinary team. The objectives outlined for the Internship with Report were: to provide care to the elderly people with dementia in a complex care situation; promote in-service training for nursing staff and other health professionals on nutrition in elderly people with dementia and describe the experiences and perspectives of nurses from a dementia unit on nutrition in elderly people with dementia through a qualitative study. In the study, 7 nurses from a dementia unit in Lisbon participated, semi structured interviews were performed, and the recorded speeches were analyzed using the content analysis technique. As main conclusions, the following stand out: feeding elderly people with dementia depends on an individualized assessment and a personalized plan; the nurse is the main actor in feeding strategies and the link between the multidisciplinary team and family caregivers; the need to disseminate international guidelines on nutrition for elderly people with dementia within the nursing team and to increase their discussion.
